Going in I had: Almost all upgrades (missing Geth Shields 2, Widow for Legion and Biotic Damage 5). Full Loyalty.
I had Thane die because I made a poor choice and sent him into the tubes, I thought he'd be ok but the doors jam and he gets shot in the head.
Garrus for both Second Squad Leader.
Grunt for the Escort.
Samara for the Barrier.
No Mordin death.
In all honesty I think the game very carefully calculates just how prepared you are going in and if you aren't maxed out it puts some taxes and if you make a wrong choice for a job it guarantees deaths. I'm fairly sure time doesn't affect it, because if it does than on Insanity you would basically be guaranteed to lose quite a few people. (Enemy armor/hp/shields scale immensely while your damage does not, causing the time to kill a target to skyrocket. To see this in action simply make a new game on Insanity and do the first 2 missions.)

Ok, I reloaded from the abduction.
This time, I sent in Legion instead of Tali for the pipes.
Miranda led the first time.
Garrus led the second time, Jack biotic shield.
I let Garrus escort the crew back.
For final battle I took Mordin and Miranda.
Everyone lived, and I got Kelly too, woot.

I think it might have something to do with Mordin's personal upgrade from the researcj terminal... the +20% tech damage that is for Mordin only.

Nah my mordin was not upgraded and i saved everyone on insane.
Tali in the tube, garrus led both groups, zaeed escorted ppl out, samara biotic barrier.

Everyone was loyal for me and I sent Zaeed back to escort the crew members to the ship, I had Garrus leading the other team and left everyone but Samara and Legion at the last gate to hold the collectors off.

The only person who I ahd die was Thane and that's apparently because while he's good and crawling around tubes and ducts, you shouldn't send him to open the doors. I had Zaeed escort the crew abck as well so that means Mordin was in the fire fight and he lived.

I'd completed the game with everyone alive (actually I beat the finasl mission several times the same way and with everyoune staying alive), but later decided to replay some missions and reloaded a bit back. at the same time I installed the DLC with Zaeed. When the time came for the suicide mission, i was level 29, with full squad loyalty, with almost every relevant upgrade done (anyway, much more than the first time). I arranded everyone's assignments just the same way I did earlier, but ooops... The only one who didn't make it is Mordin... Very frustrating and quite strange. have to retry it different way now...

Don't send him into the vents
Don't take him with the biotic bubble team, if you want to though only if the person creating the field is loyal.
Then send him back with the doc and Kelly, if he's loyal. Or take him to fight the reaper/person. Don't leave him to hold the line.

There's either random chance or time-based element with regards to Mordin. I think if you select the "We need to hurry" option before you leave to kill the Reaper, you are put on a timer. If it IS a timer, however, I'm really disappointed (and kind of annoyed) that they didn't include some sort of auditory clue that you needed to hurry up, like maybe a radio call from the team or something. I'm just pissed that I had everything upgraded and everyone loyal and Mordin still died for *no* apparent reason.

Legion went through the vents. Garrus lead the other team both times. Samara was my biotic shield. Tali and Thane were in my squad.
Had no problems whatsoever.

Everyone lived on my team w/o any problems. Everyone was loyal except jack who i couldnt use the paragon/renegade option on. It's not really random since i did the same run through a couple more times and with the same results. Maybe the class chosen influences something, but idk
