1; Have you done her questline about the past? Have you/she talked enough about it afterwards? Check DA wikipedia för information about it.
2; You only get access there when trying to get evidence against Loghain, just play on, you can't miss it once it's open unless you sleepplay...
3; Shale comes from an optional DLC, why on earth whould ge be required? No, he isn't needed.
