In the Fade inside of Morrigan's Nightmare, after facing Flemeth and beheading her (which, of course, is random), I clicked on Morrigan before the game figured out the fight was over (which always seems to take a while), and it restarted the confrontation of her fake mother/ Her head, however, was still unattached, and the conversation simply proceeded without it.

Sadly, I did not get a screenshot. If someone can attempt to reproduce this, it really is a hilarious image.

Sorry, didn't think about it. It's such a peripheral part of the quest, that it did not seem particularly spoilerish
