Ok,
I have ran through ME2 3 times working on getting the no one left behind achievment. I have everyone loyal with all upgrades, except Zaed. He survives up to them going to the ship, however I beat the boss and on the way out Joker says everyone has survived no casualties. However the cutscene shows Zaed dead. Is this supposed to happen? I personally dont think he was even encoparted into the ending but still counts towards the achievments. Any Ideas?

I have everyone's loyalty but Zaeed's (won't unlock). I did that my second time after Jacob died being my tech. I also did all the paragon options during the mission.

Oh, I have the order wrong. I did Garrus as leader first, then Miranda.
