I completed the Normandy crash site DLC and everything was fine. Later I was exploring I accidently landed back on the same planet without realizing it. I walked around for a few seconds outside and then stopped. Without moving or touching controls, down I went! Fell right through the ground and died. classic.  

Anyone else? And what other games has this happened to you in? Definitely happened multiple times when I used to play WoW.
