Yeah Mordin was the only one to die on me, looks like I will have to replay it.

And also load back a save game just after my crew got kidnapped, now that I know that I could've saved them...

does mordin always die? wow im gonna send him to escort in my 3rd play

He doesn't always die..that's the thing.  I'm beginning to think there's some factor that goes through the end mission, that gets selected at random.

As far as I've been able to determine, these are the critical point choices, for squad survival:

1. Normandy ship upgrades.  Get the SHIELDS, GUNS and ARMOR upgrades.  Talk to Garrus to get the GUNS upgrade.  Talk to Jacob to get the ARMOR upgrades.  Talk to Tali (I think) to get the SHIELDS upgrade.

2. Shaft:  It's either Legion or Tali.  Anyone else will result in a death.
3. Team leaders: Miranda or Garrus are the safe bets, but I've heard of people managing it with Jacob.
4. Biotic Protection: Samara or Jack are the safe bets, but once again, I've heard of Jacob making it, too.
5. Crew member escort:  Here, as far as I can tell, so long as the crew member is loyal, no casualties.  Also, apparently the ONLY place that Moridin is sure to survive.
6. Don't select "We need to hurry" during the last speech option.  It increases the chances of someone dying.

mostly right, except i wouldnt use jacob for the biotics, as i have heard of him failing on numerous occasions. also not everyone will survive with the survivors. the best bet is jacob or grunt. mordin can survive on that mission, but its a slim chance. it is more than likely that he will die.

honestly i think the best bet is to breakdown all the choices i made, because all my guys came out okay. I upgraded the ship to its fullest, had all loyalty, picked up some better guns, then i went on the mission.

my choices were:

Tali-Pipes

Garrus-Team two leader

Samara-Shields

Garrus-Team two leader

Jacob-Escort

the reason we need to hurry is bad is because it says to the computer that the longer it takes for you to finish the human reaper, the higher the chances get of the team suffering casualties

I used Mordin for the fight at the end, and he survived.. So it's not 100% that he has to be an escort. This was my setup:

All upgrades

Pipes: Tali

Fireteam: Garrus

Escort: Grunt

Biotic: Samara

Fireteam 2 Leader: Garrus

Everyone lived, including all crew members.

My first play I screwed up by making Garrus go into the pipes and Thane be the fireteam lead :|

I've also had a friend who lost Tali during the Biotic segment, and he was using Samara for the shield... Loyal with them all, and she still died. I don't understand it.

My Than died because I sent him to go crawl through the pipes since I figured he would be the best for that but apparently I should have focused on the tech part of that assignment. Everyone else lived.

exact same situation for me. from what ive read mordin loves to die if you dont take him in the end. not much of loss if you dont take him, he is great for all the husks and no teammate seems to do anything to the final boss. luckily i took him before i knew that, after making the mistake of appointing him team leader (youd think a genius could coordinate better, but he had to go and get thane pot-shotted.)

I sent Mordin back to the ship and everything was fine. The only mistake was Jacob died, but my power went out so I got to do it again. :D

For me: Mordin got shot in the face. Grunt was carried off by the swarm. And Jack was shot a few times and died. The only one I really cared that dies was Grunt and even that was like meh, ok he is dead. Mordin got what was coming to him even though I support Genophage and Jack was just a giant c*nt. I actually replayed the last level just to make sure Jack gets killed. My second playthrough I will probably trade Zaeed's life for Grunts, which is what happened my first time doing the last mission, till I realized what would occur and thus tried to get Jack killed.

All non-playable crew members were saved.

I read here who dies is based on their loyalty rating but IMO all my characters were loyal; at least it said they were.  Because ME2 does not have a relationship bar like DAO I have no idea HOW loyal they were, just that they all seemed to like my character and respected her in dialog.  I played a good character that never sided with one or the other in arguments, always using the good faction option to solve disputes.  I don't know what more i could have done to save these people, but i am ok with party members dying.  Its war; that is what happens.  I am glad some died; it made it feel more real.

I lost 4 of them: Thane, Legion, Jacob and Jack.
the biggest sorrow for me was losing Thane - my shepherd and Thane had a relationship. but I'm asking myself the same question - how did THEY died??

I was disappointed by my ending. I did all the loyalty missions and got every upgrade. I had Miranda lead the diversion team and Tali go in the vents and that was fine. I had Grunt bring the crew back to the ship and that was fine. I had Miranda do the biotic shield and Legion and Jacob come with me through that part though and Legion got carried off by the swarm. That frustrated me to no end because, as a synthetic, the swarm shouldn't even have been drawn to Legion. (And I felt like because I did the radiation blast instead of the bomb and he's not alive there should have been a chance to recover him.) Then I brought Garrus and Miranda along with me to fight the reaper and they survived and gave the speech and Mordin is apparently dead but I never even saw it happen. I assumed people I left behind would die and definitely not anyone I had in my team. No one else here has had the problem with Legion being carried off? I wish Morinth or Jack had died...

Gave mix of Paragon and Renegade speech. Shep was ~90% of Renegade bar, ~45% Paragon. I play as I would play it if I was shep. End up mixing para and renegade.
Told team to hurry up ;s
All Upgrades/All Loyalty + Killed Reaper very quickly. Infil Assassin + OP Sniper rifle + Laser
Do not understand mechanics of team survival. Lost Legion and Tali. REALLY don't get it.

Legion : pipes. Dies when trying to shut the door at the end. Shot by random collector. Plan to use Jacob next time as he has Barrier.

Zaeed as team leader on first go because he is a seasoned soldier-leader. Miranda and Garrus with me. Zaeed keeps people alive.

Tali leads diversion team while Samara goes as biotic, once again Garrus and Miranda with Shepherd.
Tali dies when trying to close a door. Dies to gunshot by random collector (deja vu). Using Grunt next time because he wont die to being shot. Krogan make good diversions.

Sent no one back with crew. They die. Kind of expected this but needed people to hold the door. No one dies here. Mordin lives!

How is it possible for shep to die? Only chance I saw was when he saves Miranda when reaper dies.

Tali died when I sent her through the pipes - I expect because I lost her loyalty in the Tali/Legion confrontation. Apparently 75% Renegade isn't enough to resolve the conflict. I used Miranda, Garrus, Mordin, and Jack for the other roles.

your team only dies in result of bad leading. use your brains in the choices you make!! miranda is trained as a leader and tali & legion are the tech experts. Crew guard doesnt matter as long as it is a loyal member. samara or jack can hold the shield. miranda could too but you need her to lead the second team. do not place zaeed to lead.. he always gets people killed, he tells you this many times.

It isn't random. Each character has a "Hold The Line" score that comes into play during the last segment. Leave Grunt, Garrus, and Zaeed behind during the final fight.

No clue for Legion if he's loyal. Take to long on opening the pipes? Jacob would probably be worse.

I'm amazed Zaeed worked, I've heard he's not good for team leader (I've always gone with Garrus for both teams leaders in two playthroughs)

For last one, don't take diversion team with too much emphasis on diversion. Even if Grunt is a good distraction, he shouldn't be a leader. Went with Garrus on this as well.

I've completed this game 4 times and never had anyone die. ;\\\\
I tend to use the same people everytime though..

Last time I had Miranda and Mordin on the last mission just because you get abit of extra dialogue with her.

Tali or Legion in the pipes

Miranda or Garrus as leaders, you can use the same one twice ..

Shaft: Legion
First team leader: Garrus
Biotic protection: Samara
Crew guard: Jacob
Second team leader: Garrus

I think my dialouge choices speech wise were paragon, renegade, renegade (despite being a paragon)

Everyone was loyal. I took Garrus and Miranda to the terminator.
Everyone lived, Mordin even dodged a shot at the line.

every upgrade and every squad member is loyal

first time:

tali in the pipes gets killed

mordin leads the second team gets killed

send no escort and the survivors died

second time:

legion through the pipe

garrus leads the second team, starts a firing squad so legion lives

samara does the biotics with legion and whoever and no one is taken by the swarm (i wanna look up the cutscene for this)

grunt leads the survivors to safety

beat the reaper but then see mordin die anyways

third playthrough:

everything hte exact same except brought mordin to the boss battle and he survived the rubble falling on him

Thane died because he wasn't loyal I guess, however you can make him survive by leaving the best defenders (Garrus and Grunt) "holding the line" with him (don't use Thane in any part of the mission). It's also the reason why Mordin died
