So, I worked hard to get everyone loyal - did all their little missions, made sure I upgraded them up before I made the 'one-way' trip to the Omega 4 relay, and Legion ends up getting killed while trying to shut the door.  No way to save him, just *BAM* shot to the head, dead.  And with it - my 'everyone comes back' achievement.  Is THAT a big enough bug?

hmm.. That same thing happened here.. I made sure I got him loyal.. Question tho.. What did you do on the Geth Platform? Destroy it or Reprogram them? I destroyed it.

Reprogrammed them, following the Paragon path. I tried playing through the start of the last missions, and got the same result. I am done playing until this is fixed, as I am -really- disappointed.

"- Miranda and Jack will have fight after Jack's loyality mission - must have high lvl paragon / renegade to keep them both. Same with Legion and Tali after Legion's mission."

Just found that in another thread.. That might be why...

Had both of those moments - kept their loyalities, people made up.

Not sure if this is true, but I've heard there's a random chance for anyone to die even if you do everything right.

That being said, there ARE combos of people you can pick for each task that are vastly superior to others (i.e. ppl don't die)

So.... since I didn't pick one of these 'dream team' combos that I didn't know I had to - I am locked out of getting that achievement... and since I didn't make a seperate save BEFORE getting into the Omega 4 area... Well, shame on me. That really hurt my chances of getting the third installment at this rate.

Kind of, but the choices are fairly obvious.  For the tech specialist, you pick your best tech characters (Tali/Legion), for Leader, pick a good leader, etc.

Also, there should be a "restart the mission" save that will let you do the whole thing over.

Legion as my Tech guy

Garrus as my 2nd team leader

----------------------------------------------

Samara as my Shielder

Garrus as my 2nd team leader

Tali as the escorter

Miranda and Jack were in my Team the entire time.

Only Legion died.

Sadly, the 'Restart Mission' save brings me back RIGHT AFTER the decision of who is on what team is already chosen.  The only choice I can make is who goes with me on my team.

Miranda is a better choice as 2nd team leader.  I've played through the entire game twice with different imports.  Both times I've gone with Legion for tech and Miranda for 2nd, and both times had everyone survive.

I had Tali as my tech, Garrus as my 2nd leader.

Then had Samara as the shielder, Garrus as the 2nd leader and Jacob to escort the ships crew back.

I had Miranda and Thane with me the whole time and at the end everyone was saved.

If you miss some ship upgrades then some characters will die during omega 4 jump. If you pick wrong people for different tasks they will die. if you have not loyal people in a team they might die.

So get everyone loyal, make ALL ship upgrades and pick appropriate team members for the tasks like Legion for hacking, Thane for taking a vent, Grunt for escort, Garrus for second in command and Samara as biotic. It worked fine for me. But of course there are different combos of chars.

They flat out state you need someone with tech expereince for job one and someone who can lead from experience for job two. And considering you know there is an achievement for everyone survivng, you should have rightly took that into account.

If any of them are tricky its the second set of choices about what biotic to bring, seeing as how Miranda flat out states theortically anyone with biotics can succeed at it.

Weird. In my first (and so far only) attempt, I used Legion in that role (along with Garrus as my leader both times, Jack as the biotic and I believe Zaeed as the escort, as well as Tali and Mordin in my party the whole time) and everyone lived. The escort could have been Jacob, but I'm pretty sure I picked Zaeed.

I have tried sending both thane and the professor down the tunnel and they both died, I want everyone to live as I am very paragon and want to play ME3 with a surviving crew. WTF? Everyone is loyal etc. Glitch or not? I saw someone else say they sent thane down the shaft and he lived. For those of you who have this problem: Do you have the Ish quest glitch as well? If so, they may be connected.

This is not a bug, The person in the tunnels survival is based on:

1: Who it is

2: Their Loyalty

3: Who is leading the fire team.

Yeah, you are correct. I went back and sent Legion through the tunnel because he's a tech specialist (all are loyal) and everything went swimmingly. I had Garrus lead team 2 both times and had Zaeed escort the survivors back to the Normandy. Yay, no one left behind.

You have all the equipment upgrades like geth shield boost and damage protection?

Personally I had legion escort the crew back to the ship. That's the one surefire way to keep him alive that I know of.

My setup was:

Ventilation Shaft - Legion
Fire team leader: Miranda
Squad: Tali'Zorah / Grunt
--
Survivor escort: Thane
Fire team leader: Miranda
Biotic escort: Samara
Squad: Tali'Zorah / Grunt

Having all the upgrades does make a difference, as well as loyalty.  In my instance, I had all upgrades and all were loyal and sure enough, everyone survived.  It's not a bug that Legion got shot in the OP's game, but it was kind of a mistake to not have a save point before going through the relay.  It is, after all, the entire focal point of the game (preparing to go through the relay), so it would have made sense to save just before going through.  Don't worry. That's what second play throughs are for though, right?

Upgrades factor in as well. You can pick the right people, with loyalty and still lose someone depending on ship upgrades... from my exp at least.

Has anyone lost Shepard? In one promo talk they said Shepard could die, permenatally. I'm actually playing for this on my next go, a speed run with trying to see if Shep will sacrofice himself for the team. Speed run, should be interesting how fast you can finish with doing just the bare min, not skipping conversations but not doing loyalty quest or even picking up the full team.

I would reccomend upgrading as little as possible(especially don;t upgrade the normandy or personal defenses),  leaving a few squadmates to recruit after the IFF mission, don't make anyone loyal
