I took Shale into the Anvil of the Void so I could do his quest. After I beat Branka if I right click to engage Shale, I just get the activation question for some reason where I have to say the phrase.
Also background goes really black and it looks like it takes me outside the map. I can still see my characters, but I can't see where I'm going.
It doesn't matter where I do this. Even in camp it happens. I bascically stopped using Shale.

Sounds like the Disappearing DLC glitch reset Shale. See the DLC In Game Errors FAQ for more information and workarounds for how to get the DLC back into a good state.
