You need to have your ship fully upgraded with like shields, armor, weapons, etc.
Heres the layout you need, assuming you have all ship upgrades, some squad upgrades, and most loyalty (if Zero is pissed at you it wont matter here):
Tali for the vent
Garrus for squad lead (both times)
Miranda and Grunt for 1st squad
Tali and Mordin for other 2 squads
Samara as biotic escort
Send Jacob back with the crew
Try to pick any dialog options that say something like 'hurry'
Do this and youll get through every time wwith everyone

I believe it goes that your birth ship is replaced with the ship you're accepted with. That's why they refer to Tali as Tali'Zorah vas Normandy, and not Tali'Zorah vas Normandy nar Rayya. So, I'm not for sure, but I guess if they did get hitched it would be Tali'Shepard vas Normandy.
