WTF!!! I'm getting SOOOOO ticked off!!! end game spoilers for ME 2

So it's like this, my first runthrough of ME 2 I had full loyalty with everyone and full Normandy upgrades, chose Legion to go into the ducts and Samara to lead the first team...ad legion died.

Fair enough, I thought I must not have talked to him enough, as no one else in my team died and I had gotten him last.

Then on my second runthrough I realised something must be wrong.

I dont have full loyalty ith everyone but I read in the book that you only need full loyalty for the people you chose.

Had full upgrades for Normandy again and noone died in the crash.

Chose Mordin to go into the ducts and Garrus to lead.

Mordin died,

I'm like wtf? so I reloaded to BEFORE the crash and chose Garrus to go into the ducts and someone else to lead.

Garrus died.

SO AGAIN I reloaded, THIS time I chose Jacob to go into the ducts and Miranda to lead.

JACOB DIED!!!!!!

Now by this time I am getting really aggro, I don't understand what is going on!

ALl the people I chose I had FULL LOYALTY with and its not like I took forever, I got through all the ducts in about a minute, sometimes beating them to it.

And always the door gets stuck and ALWAYS someone gets shot in the head.

Tell me am I missing something? Maybe theres a gltich in my game or something because I dont understand what is going wrong. If I cant get past this bit then I cant get the achievement for everyone surviving.

Ok rant over, sorry for the length. I hope someone can help me.

Actually that's intentional. If you don't "pick the right person for the job" people die, thus the achievement for everyone surviving. I won't post the correct choices to get through with no casualties to the board but my good friend google is a filthy gossip, ask her about "No One left behind" and she's sure to have something for you.

edit: And before you ask, of course google is a girl, what man could organize all those search results?
