Well, that only took me a single play-through to find problems. Not inspiring, Bioware. Especially given the extra time you took getting it debugged.
It's not a major glitch but it is one that will force you to reload a previous save to get out of. In the tunnels segment in the second room where you encounter corrupted spiders (there are three of them in the room I am speaking of) there is a section in the center of the room with a bunch of wood planks for flooring. If you get attacked by a spider while standing on them (or use an attack ability, I'm not sure which triggered this) the game could glitch you through the floor into the room below you. Regrettably, said room below has no exit whatsoever so once down there you are permanently stuck there till you load a save from before that room. I was most curious about the glitch room because it was actually a completely different room from the one above it. It had lots of tree roots marking parts of it off and several collapsed segments of wall. Made me wonder why it's in the game since it is not legitimately accessible and does not have an exit of any kind to return to the rest of the dungeon.
Anyway, just a heads up for players to avoid and Bioware to fix (something that should have been done before release given the extra month you spent to fix this dlc).
