This is a roundup of the most commmon bugs and complaints posted on this forum, so that hopefully Bioware's staff can check on them without trawling the entire forums, and players with problems can check if theirs is already known about and if there is a fix.
I'll periodically update it, if I'm missing stuff let me know, I'll try to get round to adding everything.

Game bugs

Squadmate loyalty skills locked after completing loyalty mission
This is known to occur with several squadmates and seems to have various possible causes. It is confirmed for example, that several users lose Grunt's loyalty skill (But not his loyalty) if they carry out the Pyjak killing mission.
Status: FIXED BY MAY 17 PATCH

Miranda/Jack showdown impossible to resolve, charm/intimidate greyed out or choosing charm/intimidate fails to retain both loyalties
This seems to be a bug caused by the order in which the loyalty missions for both characters were performed. testing has repeatedly shown that if Miranda's mission was done first it is impossible to retain the loyalty of one of the two.
Status: Glitched. Fixable by doing Jack's loyalty mission FIRST. Possibly related to the Morinth charm/intimidate bug. (Note - dev claims this is intentional for people who have made many paragon/renegade choices without focusing on a specific morality)

Events/cutscenes not triggering duing missions
Sometimes players have reported particular events or cutscenes not triggering, leaving missions unable to progress. This can include such things as Garrus remaining on the floor after the gunship is shot down.
Status: Glitched, fixable by reloading.

Javelin missile N7 mission
When choosing which missile to destroy, the game chooses based on the button pressed, NOT on the option highlighted. This can be misleading and has led several players to get the opposite result to that which they intended. Also, WHICHEVER choice is selected, the Journal reports the opposite as having taken place.
Status: Glitched. The journal will always display the opposite of the choice made. Word from Bioware would be good on this matter, as it may affect ME3.

Conrad seems to have a bad memory
Whichever choice was made in Mass Effect 1, he is always convinced Shepard pulled a gun on him.
Status: Glitched. Has anyone managed to get him to talk as if the paragon dialogue was used in the first game? Feedback would be appreciated.

Off-hand ammo pack not working
This seems to be caused by picking up a weapon or accessing a locker during a mission. If this is done likely a variable is reset which results in the increased ammo capacity no longer applying.
Status: Glitched. Avoidable by retaining your originally selected weapons.

Sound cutting out/distorted/etc
Several players have sound cut out, sometimes during specific missions other times at random. Some players report dialogue being distorted to an unintelligible degree, while others have noted bugs such as a male Shepard speaking with the female voice. Sound cutting out seems particularly prevalent during Samara's loyalty mission.
Status: Glitched. No fixes or cause identified.

Camera angles in cutscenes
Often during cutscenes the camera will get lost in a wall, or inside a character's head. Sometimes a paragon/renegade interrupt will come up but the player will have no cue as to what it is suggesting because all they can see is a shoe, or a piece of blank wall. Also characters can randomly teleport around during these scenes.
Status: Glitched, reloading often fixes specific incidents of this.
(As a sidenote, does anyone remember the hilarity in Final Fantasy VII if you took Yuffie and Vincent with you when Aeris died? The way Vincent's animation would glitch and he'd start goose-stepping sround the room?)

Controller disconnecting at certain times
Certain players have reported a glitch (specifically in Jack's loyalty mission) where the controller will disconnect for no apparent reason while still fully powered.
Status: Glitched. A fix seems to be to disable controller vibration.

Shepard locking facing in a single direction
Often during fights, Shepard will become stuck facing in a certain direction, away from the direction the camera is facing. Moving the right stick will rotate the camera, moving the left stick forwards will have Shepard run backwards or sideays. Firing a gun will always fire in the direction the character is jammed facing in. Using storm will make Shepard run in the direction being faced in rather than the direction the camera is pointing.
Status: Glitched, leaving an area containing enemies then firing your gun while none are nearby often locks Shepard's facing back to the camera control.

Mass Effect 2 unable to locate Mass Effect 1 completed game saves
Many players find Mass Effect 2 unable to locate their Mass Effect 1 saves.
Status: Possibly glitched. Sometimes a result of changing gamertag. If this is the case, move your saves, using your new gamertag, to a memory unit, and they should be reassigned to your current gamertag. This may also be due to game updates, with saves pre-patch being undetectable. Some players are investigating this.

Tali's Trial scene not occuring
If you talk to Shala'rann before getting close enough to activate the trial, the game treats it as if the trial has already taken place (Shepard getting angry over things said in the trial, etc). After this point you will be sent to the ship without knowing what should have occured during the trial.
Status: Glitched. Fixable by reloading.

Ish mission in journal not resolving
If the paragon choice is made to end this mission, after the credits and experience are awarded, the mission remains open in the journal.
Status: Glitched, only way to end the mission and have it resolve in the journal seems to be by giving the data to Aria's Batarian.

Inability to save
This seems to occur for some players after completing the introduction and setting their appearance.
Status: Glitched, cause unknown. Advice is to attempt to save the moment you complete this part of the game. If it does not allow you to, restart.

Disc read errors
Sometimes a 360 is simply unable to read a clean, new disc.
Status: Cause currently unknown, no common factors yet determined. May be connected to game freezing bugs some players with the game installed to their HD are experiencing.

Minor dialogue glitches
Legion mentions events on Virmire as having occured on Ilos, some of Thane's dialogue is subtitled as being spoken by Joker.
Status: Glitched, but not game breaking.

Miscellaneous clipping bugs
Several areas of the game have clipping issues where enemies can fall into geometry, or the player can get stuck (Outside the Normandy, for example)
Status: Glitched, fixable by reloading.

Flying squadmembers
This glitch can result in team members becoming trapped in geometry. Apparently also sometimes occurs to Shepard. Sometimes reproducable by sprinting into a squadmate.
Status: Glitched, possibly a collision issue.

Controller sensiivity too high
Some players are experiencing poblems with the controller sensitivity and drift.
Status: Requesting input from Bioware. Apparently fixable by using wired controllers according to some players.

Tali mission - PRESS B TO END MISSION plastered over dialogue
In
the post-mission sequence where you get a lot of important dialogue,
the "Press B to end mission" notice obscures the dialogue selections.
Status: Design oversight.

Samara Loyalty Mission - dialogue not unlocking
Many players with 100% Paragon/Renegade either experience greyed out charm/intimidate options, or NO charm/intimidate option when in Morinth's apartment.
Status: Glitched.

Squad ammo powers overwriting selected one
Squadmate ammo powers constantly override the power selected by the player and will be repeatedly used by the squadmate.
Status: FIXED BY MAY 17 PATCH

DLC bugs

DLC locked because of Live/EA/Bioware accounts not being properly linked
EA are aware of this issue and are fixing it.

To check and see if your xbl gamertag is linked to your EA account:
1. Go to support.ea.com
2. Login into your EA account (the one you used to register the codes)
3. Click on "My Account"
4. Scroll towards the bottom of the page that opens and look for a section called "Select a Default Persona"
5. If your gamertag is linked to that account, it will show up there.
6. If it's not, you can CHOOSE to link it by clicking on the button: "Add Persona"
7. Then follow the instructions
- or -
You can use their live chat or call their number directly to get them to do it

Phone number (US): 650-628-1001

DLC not working offline/Game signing out of gamertag when Live connection is lost/etc
It's not certain right now how this is intended to work. Several players report different experiences. some can load their game offline with all DLC enabled, some require constantly being logged in to Cerberus, others can play with DLC if they were logged in to Cerberus when they loaded the save, and continue playing if their connection goes down.
It is possible those who need to constantly be logged in have an issue with the DLC being recognised as registered to their console.
Status: Awaiting a response from Bioware/EA  on how this is intended to work.

Content restricted due to age - on an adult XBox Live account
This bug appears to possibly be related to former child accounts which have been upgraded once the individual reached the legal age in their country. sometimes the network does not seem to recognise that the gamertag should now be able to download mature content. Seems to be isolated specifically to Mass Effect 2 DLC.
Status: Possibly a cerberus glitch.

Shadow Broker Bugstravaganza!!!

DLC not loading at all
Several players never receive the message relating to data on the Shadow Broker after installing the DLC. This occurs even if Ilum is unlocked. No message appears at their terminal and no new conversation options arise with Liara
Status: Glitched, no explanation/solution yet

Game locking up during first conversation in Liara's apartment
For some players, the conversation in Liara's apartment crashes the game, this problem appears to be unavoidable for those with the bug, and deleting and redownloading has mixed results. This problem occurs on 360 AND PC
Status:
Glitched, no explanation/solution yet

No sound/Broken sound on car chase
The audio often cuts out or is not present at all during the car chase.
Status: Glitched, no explanation/solution yet. Possibly linked to faulty audio channels relating to the car's thrusters.

"The Hard Way" achievement impossible to get
When beginning new sections of the DLC, the game randomly toggles the difficulty to Casual, with no notification. The only workaround is to restart the DLC from the beginning, but this is not guaranteed to work as the bug will likely occur again.
Status: Glitched, some players report that manually setting difficulty to insanity at the beginning of each section fixes the bug.

Miscellaneous complaints

Text size
Many users of SD televisions, and also some HDTV users report that the text fonr used and/or the lack of contrast, means that they are unable to properly read text. A bolder font or text shadowing may help with this issue.

English dub for German players
Many German players bought the game having been assured of zero-day voice pack DLC to replace the dialogue with that of the original English version. This has not been forthcoming.

Curiosity about a possible bug: Consort news and Siirta news
Some players are wondering if there was supposed to be dialogue referring to the consort NOT leaving the citadel due to rumours of information leaks, if the player stopped rumours from being spread in ME1. Perhaps PC owners could look into this by checking the audio files for the news announcements.
Likewise it menions another mission in ME1, one which ITSELF had a glitch which may cause this result, and which DIRECTLY relates to Journal bugs such as the Javelin one. Specifically, no matter what ending the player got to the Siirta biotic attack mission in the first game, the first game's news reports always referred to it as a massacre. Likewise ME2 has news reports which always treat this as having occured regardless of what actions the player took. This implies that any similar bugs which result in the game recording the wrong outcome in ME2 will carry over to ME3....

Will do. Several friends of mine had this problem. Off to university right now but it'll be in my next update.

Some people - including me - have the bug that in cutscenes and dialogues NPCs suddenly disappear, reappear out of nowhere, change positions, look in different directons suddenly etc.

The news report about the Consort leaving because of political scandal. I don't know if this is a bug or if your actions didn't aid her during ME1. It could be there were scandals after that.

Bugs I've noticed that are not on the list (or not mentioned in the replies already).  Some of these are more significant than others of course:

-It's possible to skip Tali's trial during her loyalty mission.  If you talk to Shala'rann before getting close enough to activate the trial, the game will act like the trial has already taken place (Shepard will yell at her for not telling Tali about her father, etc).  As far as I can tell, there isn't any way to actually trigger the trial after this point, and you're basically asked to go strait to the Alarei.  Not sure if there are any implications to missing that, since I noticed the problem right away and restarted the mission.

-Legion has a dialog mistake.  When talking about Sovereign, he quotes a line from ME1 "when Shepard met Sovereign on Ilos"... Shepard didn't meet Sovereign on Ilos - that was Virmire.  Not sure if this one is actually fixable since they'd need to bring the voice actor back in to re-record the line.

-During the last mission (right near the last fight), there is a dialog from Thane (possible this could be others though since anyone could have died, not sure) that is sub-titled as "Joker" speaking.

-If you complete Ish's mission via the Paragon path (convince him to give up because Aria will find out), the mission status in your Journal is never marked as "complete", even though you are awarded XP and credits for the mission, and Ish is now gone.

-There seems to be issues with the controller's deadzone.  See: http://social.biowar...07/index/865803

Those are the ones I've noticed.

I managed to clip Shep out of the room directly across from Morden's lab (about where the platinum is found) and outside the station (nice view). Required reload.

Also in the red hallway off the downstairs Afterlife room I clipped into the ceiling. Managed to get back down without reloading.

There seems to be clipping issues in a lot of places to be honest.  I haven't noticed any myself, but I've heard a lot of people saying similar stuff.

ME1 was probably worse for that tho - I used to get stuck in the galaxy map on the Normandy all the time :D  Luckly talking to Pressly would position you back outside it again.

  • When quizzed on who Shepard chose to be human rep on council, any answer is considered to be correct - going against the info on your imported save file.
  • Text is sometimes hard to read on my HDTV, when the text 'blends in' with the background.
  • I have the Ish package glitch, where I chose Paragon and the quest didn't end.
  • Upgraded versions of Warp, Incinerate and Overload do not contribute to the respective achievements, when used on the different types of enemy protection.
  • Suggestion for Cerberus Network: Instead of saying that there is new DLC available every time you go to the main menu, have it only appear after logging in and checking for any new DLC.

Imported character bug has got to be the most frustrating. My theory is that this is patch related. Pre-patched ME1 characters don't show while any saved character you've played post patch will show/import. I haven't been reading enough of other peoples issues but it was a logical conclusion for my sitiuation. I had one character that I had played with last (last 6 months or so) that showed but none of my characters from before that would show. Firing up ME1 and loading one of those "older" characters seemed to do the trick. Although I did load up the very last save for that character and finished the game so that could have an effect as well.....in fact, I'm going to go try loading a save but not finishing to see what happens...will report back.

I'm pretty sure ME1 on the 360 was never patched.

This is the whole point of this quiz, because this decision was made after the endsave was created, if i remember this right. So it's not a bug.

The Council appointment is fine, the problems with importing are possibly:
The Sirta Foundation Paragon mission bug carrying over
Conrad saying you pointed a gun in his face.

I say possibly because they could be explained in game, or it could be the information not importing right, I'd still really like to get a Bioware person to comment on those two things so we could know for sure.

I have a feeling this was a remnant of the original "quiz answers = your choices in ME1" that Bioware had promised initially for non-import new characters.   Kind of annoying they didn't keep it that way to be honest.  I hate how they start a new character out as Renegade :(

The new character isnt a renegade. He's neutral.

If you don't have enough paragon or renegade points for Wrex, Ash puts him down. The neutral option for the end game is to not save the council so that the Alliance Fleet will be at full power when the time comes to engage Sovereign.

I was under the impression that the new character also killed the Rachni Queen.  And I thought killing the Council was considered Renegade as well, not neutral (was there even a neutral option?).

You had three option "Save the Council, Focus on Sovereign, Screw the council."

in the neutral option you order the fleet to ignore them in order to not lose part of the fleet needed to take down Sovereign.

As for the queen, I'd have to do that section again but I think there was still an option to kill her with out it being just because your big ball of hate. Something like "You cant be trusted."

Thanks for adding this. A lot of German customers are affected by this issue and have not received an official statement as of now. It's particularly aggravating that many German reviews mentioned the now non-existant, optional voice pack DLC as an added bonus. The German dub is generally perceived as being lackluster, so it most likely even affected review scores.

Adding in some more bugs I've seen.

Squad mates going brain dead. Twice that I have seem my squad mates will get behind cover and then become totally unresponsive. They will not use powers, will not fire on people, will not move when ordered too, they just stand there.

Some times when told to get to cover, the squad will move to cover and then squat down behind it, not actually use it for cover (lean against it). If this happens they will try to shoot enemies but usually just shoot the wall directly in front of them.

Sometimes, when walking towards a low object, a chair for example, you can get stuck to it. You sort of jump 1 meter in air and you're stuck there. My bro stuck to ground without any apparent reason, same symptoms as my problem, you can turn but not walk or you can walk but only in very limited area.

The missile mission bug is not just "press the right button", which is explained in the screen when you do it. I've tried it several times and whichever option you picked will display correctly in the Mission Results summary, but the opposite choice is recorded in your Journal - if you save the spaceport, the report reflects that, but the Journal will say you saved the colony, and vice versa.

You know, I had them space out once on me, but I knew the reason.  Somehow a Mech I was fighting got lodged into the floor.  It was dead, but I could still select it if I moved my cursor over the area (just the name, had no health bar).  So my squad kept trying to attack this stupid spot even though there was really nothing there (and I even tried to use abilities like shockwave to see if I could dis-lodge it to no avail), and they had trouble responding to orders (or they would respond, then head back to shoot the wall).  Once I got them in another room with the door shut, they finally gave up :P
