1) When recruiting Tali on insanity I had just talked to Tali's friend and told him to stand down so I fought all the guys. Took out every enemy but I didn't want to waste my heavy weapon ammo but I was versing the Geth-scarab like thing. I got it on an angle that it didn't shoot me or even move when I shot it. It didn't even regenerate so I won.
2) For some strange reason if I resume my game in the main menu or continue after dying the game has a major stuff up, On major missions or a mission as such it restarts it and autosaves at the beginning so that my autosave now restarts complete missions. I have to save it all the time and always load it or I will lose a lot of levels, missions or loyalty of squad mates. I have got very frustrated. Is there a patch for the 360? Does it help or make the problems worse and if it does exist where do I download it? Thanks. I have experience many glitches in this game which is disappointing seeing it is such a good game.

This has happened to my Shepard several times. The only time it affected my squadmates was when they followed me up into the air. If it does happen with Shepard, you have to load an older save as saving and loading doesn't fix the issue (as I'm assuming it does if just your squad members glitch in this way). I'm mentioning this because apart from the most recent time, this didn't involve anyone getting stuck in geometry but rather they suddenly just rose up into the air.Flying squadmembers

glitch can result in team members becoming trapped in geometry.
Apparently also sometimes occurs to Shepard. Sometimes reproducable by
sprinting into a squadmate.

possibly a collision issue
So far I've had this issue in three places.
- First was on the huge crashed ship that falls of the cliff at the end. I went straight forwards at the start, past where you're meant to go, and all of a sudden I rose up into the air and was stuck walking along an invisible floor about 5 foot in the air that I couldn't get off.In this mission, Shepard is alone so at least in this case it has nothing to do with collision issues with squadmates.
- A similar error happened on the side mission where you light the navigational beacons. I rose much higher this time, my squadmates following me, and I was able to cover quite a large distance at about 20 feet in the air.
- Finally, I just walked onto a table at the start of the Reaper IFF mission. This is slightly different from the others in that I rose up onto a visible obtacle rather than some odd invisible platform in the sky. Still, it has the same result that I'm stuck above the ground and have to reload an older save to continue playing.
Just noticed another (minor) glitch. The Power Gamer achievement is complete but the level count when you view the achievement in Shepard's quarters still only says 0/30. Nothing major but I thought I'd point it out.

Also, at the end of Mordins loyalty mission I choose the 'Stay a while' option, went through a Level Up screen, and when the game resumed I was in combat mode and pressing Back would not exit it. This made it impossible to press B to end the missiuon as all that did was melee attack - I couldn't leave the room, or save, or exit. Another f'in reload.
While I'm enjoying some of the ME2 upgrade experience the amount of bugs is disheartening - in about 24 hours gameplay I've already had to replay more of ME2 due to bugs than I had to in over 400 hours of ME1 time.
And given that ME2 has been designed to squeeze every last cent out of the games' followers I'd really like to see Bioware releasing some more regular fixes, especially for the game-breaking glitches.
Gaming is supposed to be fun - when I find myself screaming obscenities at an unknown Bioware dev because their glitchy code has just wasted loads of my time I'm NOT having fun, and enough of that and I'll go elsewhere for my gaming, thanks,

All they have to do to fix this is either never have a default option selected so the player always has to twitch their thumb and actively select a choice, or make the Skip Dialog button not also double as the Select Response button.

Things already mentioned:
- Several save import continuity matters.
- The extra ammopack not always working.
- Ish assignment still open eventhough the text says the matter is resolved.
- There are loads of clipping errors, where you can walk up / in / on to stuff you're not supposed to, getting you stuck. I wish I had specific locations to mention, but I can't remember them.
- Plenty of weirdness during cutscenes, weird animations or weird angles which result in characters not being showed or clipped. Again, can't remember which cutscenes were affected, except for the first talk with Anderson. Both squadmates can glitch around.
Another had a Krogan clipped, you could see an outline, but the inside was missing, the effect you get when the camera is inside something, in this case the Krogan.
Another was a scene with Joker I think, one in the final part of the game. The camera was staring at an empty chair, but Joker was speaking.
Another one had a graphical glitch, I assume some shader (glass-type) not rendering, I'm pretty sure it was when doing Garrus' loyalty mission, at the part where you take the cab/car to the final part to let Garrus take the shot or not, when inside the car the windows will have the familiar "endless mirror" graphical glitch, as will the hallway you walk through when talking to Garrus before and after the meeting with Sidonis.
Not in the OP:
- Mordin's loyalty mission, in the final room, if you choose to stay to hack the computer for credits, you cannot end the mission.
- Tali's loyalty mission. When you're in the room leading to the hallway with Tali's father in it, you have a big fight, but you can see the body already lying in the hallway when the Geth open the door. You can skip this battle by using a drone targeting those Geths. This will immediately trigger the scene where you check up on Tali's father. Maybe the game wrongly identifies the drone as the player or one of your squad.
- Project Overlord. Game crashes when entering Prometheus Station. After some googling, this is apparently caused by having Legion in your party.
- A lot of times, when I'm behind cover, I click the right mouse button to aim, but my character won't respond untill I actually shoot. She will only leave cover if you hold down the firing button, which is very irritating. You can't track an enemy this way, unless you fire your whole clip empty.
Also, not really a bug (or perhaps it is?), but I find it annoying how some of the dialog options change positions in a dialogue loop. Sometimes an option will appear topleft, then middleleft.. making me sometimes choose the same option if I skip through some dialogue and want to select the next one quickly. Also, dialogues should generally be more logical.. It makes sense for some informative dialogue to be able to be repeated, but it doesn't make sense for a lot the other dialogue. Especially if you can choose a particular option right away. This happens a lot with "follow-up" dialogue options from a query selected earlier. What makes even less sense is that sometimes the dialogue option itself will be different, but it leads to the same conversation.
I love the conversation mechanic itself, the ring and the way it integrates fluidly into the cutscene, but the dialogue itself should be improved (i.e. "flow better") for ME 3.
Anyway, I really loved the game, but it's kind of disappointing that this game has so many issues compared to the first. I can't remember any glitches from ME 1.

- Elevator that goes up and down likea haunted house.
When I go in, if I stand right next to the wall in front of me. When the elevator goes down, I get stuck in an upper level little rail... I look down to see my guys all the way down.
Funny. Happens everytime I stick to that wall though, avoided simply by staying in the center. Still funny though
PS: Overlord DLC, can never be finished. Always crashes upon return to the normandy.

You're not supposed to pick anyone. Anyone that is still alive, not coming with you to the final battle, and wasn't sent back to the ship as an escort (for Dr. Chakwas and such) is left there to Hold the Line.JuiorFao wrote...

I have the same issue. I've only noticed it after Kasumi's DLC was available. I downloaded all DLC so far.
When I was on Freedom's Progress, the very first battle with the Rocket Drones, the final drone dies, but the target is still available. Miranda and Jacob continue to fire at the empty target. You don't have the freedom to progress and must restart the mission. This happed twice in two separate games (once as male and then as female).
Then, on the Archangel mission, just before Jaroth enters the battle, I was using shockwave and enemies flew off of the edges. I assume one of them survived because the Jaroth scene would not begin.
When I was trying to replicate the bug, one of the attempts shows Jacob walking on air.

Not sure if this is related to your problem, but isn't the incineration achievement associated with the incinerate power and not the ammo? I think I turned off squad powers and manually got Mordin to incinerate enemies to get that.

The credit reward is bugged-- you only get 90,000 total regardless of how much you find & what the mission screen says.
Even worse, you do not get proper squad points from leveling up during the mission & Liara is short a point.
Very fun DLC but horrible bugs.

This, I believe, is game-breaking. I have no other disc read errors with any other of my games and thought I may have needed to get a new xbox, now I'm kind of regretting buying ME2 in the first place.

It's only an idea but could try to recover your gamertag, helped me with dragonage when my achievements were messed up
