First is that the dialog for the cutscene of Samara's confrontation with Morinth - right at the beginning of the scene climax, when the two are fighting and arguing with each other, the dialog stopped playing, and didn't play for the whole scene. It happened to a friend of mine on his xbox, same cutscene, same problem. On a separate playthrough the dialog played correctly, no indication why.
Second, I finished my first playthrough of ME2 and then imported my character for a second playthrough. I got all the usual bonuses of minerals, medigel, etc. My medigel was set at 7, just starting the character. However, when playing the mission to escape the station, when the game "teaches" you how to use medigel and you heal Wilson, my medigel count reset to 3! For my husband's character the same exact thing happened, it's a repeatable bug. The medigel count you start with as a bonus just gets overwritten with the standard count for starting a game. It's aggravating having to spend the money and resources having to research something again that I should have had to start with. Doubly bad because I'm trying to play through on Insanity, and am going through medigel for dumb squad moves like crazy...

At the very end you can chose to leave/end mission or stay as a dialogue choice. If you chose to stay sometimes u are unable to leave VIA hitting the B button. It registers the current state as attack mode, u cant save or holster your weapon.
Thanes Loyalty Mission
Near the end when your target takes to more of his body guards, their is a door that u can enter. I entered it twice before giving Thane the update and both times it glitched out.
The first glitch was when the "stock boy" entered conversation with me, the door closed and thats all I could see. The Dialogue vanished and all U could hear was Thane telling me that I was losing the target. I couldnt do anything, everntually had to exit to dashboard and reload.
Second time I went into that stock room early and I went through all the diologue and then went back out to where the body guards where, the target vanished infront of my eyes, the stock room door locked and eventually same thing happend and I had to exit to the dashboard and reload.
Samara's Recruitment
At the very end (playing on insanity) I killed all enemies and all I had to do was read the data pad for the mission to end. Well one of the toxic canisters exploded right over the data pad. I went to read it, mission ended fine, then during the loading screen my controller started to shake like crazy. Loading ending and I was then dead flying around the Illium background
I think what happened was during the loading screen the toxic gas killed me?

Is this actually a bug? I thought upgrades don't carry over between playthroughs and medigel capacity is an upgrade (unless it was explicitly mentioned? I don't remember exactly what was on the list).

I completed the mission mid-combat, which may have been the problem? I got tired of fighting (Insanity lvl 30) armored husks so I left my squad to distract them while I sprinted to the Reaper indoctrination device to end the mission.

In the opening movie for the game is shows the Krell attacking and fighting with his hands and fists, something which ingame he doesn't actually do, this is gross misadvertising.

You're right this isn't a bug, the medigel resets to the correct level after healing wilson, you need to get all the medigel upgrades to get back to carrying 8 at a time. If there is a bug here, it is only that the game says you have 7 at the start, when it should show you having none. Or else you'd be able to heal wilson without using the one from the wall.

This is also happening on my games, which is odd because I've done these Missions before and I didn't get the bug that time. The only difference between my first time and this time is that I got the new Cerberus Armor/Shotgun DLC installed.
I ended up going with the Renegade Trigger and punched the guy to get through it.

Same thing happened to me, so I took the disc out and wiped it before retrying. Worked after that.

This is my 3rd playthrough on insanity, each having 30+ hours because I went through everything throughly. The 1st play through I did not have a problem, however the 2nd play through this is when the glitch first occured. I don't understand why it does this but, after I complete the Loyalty mission for jack, and when you blow up/ get off the station. You come to a cut scene while your in the ship, when joker tells you how Miranda and Joker are in an argument.
So I proceed to go into the room while their arguing, and your given 4 choices. The first playthrough like as aforementioned I had no problem. I was able to choose the Paragon action or speech line in order to resolve the issue, without losing the loyalty of either Jack or Miranda. And since I'm not a high renegade, the renegade option is out. I'm approximately 98% paragon, and I know for a FACT that on the first playthrough when I did this mission, I was no where near 90% paragon, yet I was still able to perform the paragon action.
The paragon action is black out making me not able to click it, and I'm pretty furious at this, since this is the 2nd time. And this is the sole reason that I thought I could rectify by playing through the story a 3rd time. In my first playthough I accidentally chose the wrong person for suicide task (going through the tunnel) during the suicide mission. Which in turn led to Mordin dieing, since that was who I chose. Therefore I was not able to get the achievement.
As such I really want this problem to be fixed, for it has left a pretty bad image for ME2, I don't want to waste 90+ hours of my time for something as stupid as this thwart my appreciation for this game.
Also I noticed this samething for Zaeeds Loyalty mission, at the end since I chose the paragon ending, I'm not able to rectify his problem with Video by choosing the Paragon dialoge line. In the first playthrough I was able to, and I was still able to have his loyalty. However since the Paragon option is blacked out, I have no choice but to pick something else. Which in turn leads to not having his loyalty, and will more than likely end up dieing in the Suicide Mission.
PLEASE HELP ME WITH THIS PROBLEM!!!

Grunt Loyalty Random Renegade Bug: Even though you pick all Paragon options, you get +15 Renegade points, as well as your 15 Paragon points. Aka, you're give Renegade points that you would have been given if you picked renegade options anyways, even though you never picked them.
MSV Strontium Mule Bug: Research Docket unresponsive, even though it's highlighted as something that's selective.

Edit: the next time I tried to give her an order she did nothing, but then her powers went grey and stayed that way like she was dead.

- The in-game achievement browser doesn't update the progress for the Level 30 achievement. It is constantly at 0, until it is unlocked.
- Sometimes the music stops during battles. This occurs most frequently when an explosion goes off near you.
- Reloading when entering cover will cancel the reload.
- You cannot move, whilst in cover, when you are reloading.
- Using a Power like Barrier will carry on into cutscenes. A blue Shepard will be in cutscenes!
- Sometimes when on the Normandy, you will see the shield regen effect on Shepard.
- Subtitles will appear too early and stay on well after the line has been said.
- No autosave notification appears when the game autosaves (PC).
- When switching to a new weapon via the power menu, you cannot switch to another one. You have to return to real-time, watch your character get out the new weapon and then switch (PC).
- Scrolling through your weapons by using the mouse wheel has a completely different order than the one shown in the Power Menu! (PC)
- When pausing the game via the power menu you should be able to queue up powers by pressing the hotkeys and not having to click them (PC).
- Quickload loads the most recent save - not your quicksave! (PC)
- What happened to double-clicking on buttons? I hate having to select the button then move my cursor down to the small button at the bottom of the screen (PC).
- Space bar should not have three functions. Separate sprint, interact and take cover. We are playing with a damn keyboard, we do not need simplified controls. Just allow more button mapping options (PC).
- Please bring back the hotkeys for journal and squad screen (and any others) (PC).
- No mousewheel support in menus, need to have this added (PC).
- Button prompts do not correctly represent the assigned button. eg. My melee button is V but the game tells me to press F (PC).
- There is a really annoying glitch with dragging items from the Power bar. When dragging an icon from the Power bar it sort of glitches and thinks you're dragging a Power icon at the bottom. This makes it tough to organise your Powers (PC).
