I purchased ME2 new at best buy and began playing through on insanity--shooting to try to get every achievement on my first playthrough.

I am currently ~20 hours in (in the middle of recruiting the Assassin) and when I opened ME2 last time, I got the "please log into cerberus network" thing again.

I logged back in, and there was no "resume" option on the main menu.  I selected "load game" and it found my career successfully.

After loading up the career and beginning play, I noticed keybinds had reverted to defaults (I switched space and shift, made Z and C move teammates, Q to form up and E to holster weapon to feel more like ME1).  I rebound the keys and continued playing.

After entering combat with some of the Loki mechs in the mission, i used Incinerate on one of them and a thing popped up saying "Incinerate the armor of an enemy, achievement 1/20 unlocked!" or whatever.

A reload of an earlier save on the Normandy shows that even though I am currently ~20 hours into my playthrough, I have zero achievements--including the ones you get just for starting the game, recruiting people, gaining loyalty, etc (all achievements I will have to start a completely separate playthrough in order to gain).

I have double checked, and I am logged into the correct cerberus (EA) account, and my game works and I am still at the same point in the story line.. The biggest issue for me right now is the lack of achievements--and the question of whether or not after I complete this playthrough it will register as a COMPLETE playthrough of the insanity difficulty (or if it will think I switched to insanity partway through/wont register a complete game/etc).  

Any ideas?  Where does the game store Achievements/keybinds/etc.  How can I fix this?

Yeah, this happens to me too. It's very frustrating.
