I get that most of the time too.. and consequently only one of my characters has ever uploaded properly to this site. I'm connected to Live and all that but it still says it can't access my online profile.

I doubt anyone cares. I've tried to contact EA and Bioware about account problems but EA actually has some anti-piracy intentions behind limiting accounts and tracking them, which in turn causes random errors while in-game, or identifies your EA ID as a unique dragon age persona aside from an Xbox Live/PlayStation Network account. So my Dragon Age account won't update properly, cause my EA ID is just stuck as my "default persona" on the bioware social network. Changing default persona on EA won't help either. For some reason it has no effect on Bioware's social network. I've probably spent 2-3 hours worth of my time talking to EA customer support and searching online forums for solutions. It took me threatening to contact "your manager", and then the BBB (Better Business Bureau) before they finally listened to me.
After fixing a few minor problems (a small part of the overall issue), EA Customer service would just respond to my questions by labeling them as "unresolved". Poor Bioware is just stuck in the middle of EA's rediculous policy. However, you'd think Bioware would help the fans out. At the very least speak to EA about the problem, or if it's getting fixed give us a status update. We just want confirmation that someone actually cares about this crap. Do you think EA customer support has some sort of training program? Or do they just hire idiots off of the street. Don't worry Bioware, I still love you. But your Mom is a total b*tch! <_<

Try deleting your character here on the site and re-starting your saved game and playing awhile. It won't delete your achievements or anything .. just the character. Usually this fixes the can't connect to server message. Your character will eventually show back up on the site.
You can also do what I did and just download the character creator for the pc. and create an avatar you like.. or a replica of your xbox one. That way you'll at least always have your own little unique avatar.

I did the PC character generator so i can at least get a portrait. I am thinking about deleting the char from this site, you are sure it wont touch the char on the xbox at all?

Happens to me as well. Cause unknown. Appears to happen randomly. It may not be related but my character history on line shows incorrect info as well.

I always get this error message with this one character I have..... The character portrait and avatar profile pictures never get uploaded. So I deleted the character's data from my online profile space.
@gberkhof Deleting character data on the profile space does not alter the files on your game console at all.
Did this fix my problem? Nope. I waited in vain for weeks for the character's data to show up, and as before, only the XML [story+stats] files were uploaded. I read somewhere that you should do a "hard save" ie a fresh save slot vs saving over an existing save game after deleting the online character data. I don't know if I want to bother doing that again.
Like everyone else, I find this trouble very annoying. I mean, the bits of data being sent isn't that much, just a total of 4 files at a time. XML files are just bits of code written in text and the pictures? But then I realize that PC users can have screenshots automically uploaded also. And....... I read that the DA:O is certified platinum--not one, not two, but three times! Something like ~3.2 million copies sold worldwide
. No wonder there are access errors.
This online profile feature is awesome. If only it works perfectly. I really enjoy looking at my story progression on my profile page. My TV is native 480 resolution so reading text on it is a real chore and it really strains my eyes so I can't enjoy the reading Codex and the story progression on my profile is an adequate replacement. Until I can afford a TV with high resolution screen. For now I'm stuck with text that is surrounded by moving, coloured pixels on screen. This is 1000x worse than normal jaggies with anti-aliasing off....
