Thats why you save beforehand. Interrupts are spontaneous, paragon or renegade. Like you said, not many of them have foreshadowing but this particular interrupt was pretty predictable considering the context established from encountering her in the first ME.
To be honest I doubt a player should have to save before every single dialogue. That's simply bad design...
