Legion and Grunt are INSANE together. But Grunt always goes with me. Even as just a pure meat shield, there is none better. Legion's got that shield regen which helps keep him up too.

Miranda has the best skills for every encounter. Overload for shields, Warp for armor and barriers (not to mention preventing health regen), and Slam for single target damage. Her passive bonuses are also some of the best.
The rest are all pretty situational.
It should be noted that if you want a fun party setup, an Infiltrator (Reave as a bonus) with Garrus and Thane is quite enjoyable.

I tend to take along a combination off Garrus, Grunt, Tali and Miranda. Unless its a loyalty mission where its required to have at least one off the others.

With my vanguard, I've found Zaeed and Drone-specced Tali to be extremely effective.

Completely situational, but my standard team was Tali and Samara with a soldier Shepard. I like me teams well rounded.

Miranda and Grunt is a great combo with my infiltrator in general. When fighting geths, I enjoy using Tali.

I usually tale Garrus and Miranda. I'm only starting my Sentinel on Insanity, but I can already see that I'll need Garrus all the time (because he's got the sniper rifle, and another Overload).
But I would argue that there really isn't much better than Legion with the Widow and Shield Boost. That guy takes care of anything. I might get him together with Garrus later.

Zaaed and Grunt. Or Garrus and Grunt. Or Samara and Grunt. Basically I fail to see the point in leaving the ship if you're not packing an assault rifle. It doesn't make much odds on the easier difficulties I guess, because they are easy, but on hardcore the stopping power at distance is vital.

I love using Grunt for combat. The fact he can fire a gun AND ram people -and take huge chunks of their life away with it- physically if they get too close, it makes for something useful. Also his Fortification ability has saved him AND myself time after time again on Insanity difficulty.

For me, Grunt is the most useful companion. He doesn't die. Everyone else seems to, quite often. I can count on Grunt to survive and use his Concussion shot to bring down shields without me having to rez him every few seconds.

I play only on Insanity and I find having Garrus + Thane a lethal combo through the entire game.
Jacob is near useless on insanity, he just can't deliver enough damage fast enough before you get overrun by enemies. His pull isn't needed.. he lacks skills to deal with armor, shields and barriers (incendiary ammo won't cut through armor fast enough on insanity using just a pistol)
Miranda is quite strong once you get her warp + overload skills up (and a tempest smg). A strong all around character.
Grunt can live a long time but does horrible damage.. he can't kill anything in a reasonable amount of time on Insanity. However, on missions with husks, Grunt is very very very good to bring. He almost never dies from being swarmed and his krogan charge 1 shot kills husks even on insanity. In ranged combat though, he just lacks sufficient firepower.
Jack is useless on insanity.. shockwave is nearly nullified due to all enemies having armor, barriers and shields. Warp ammo would be useful if she had an assault rifle or SMG... but she can't do enough damage with a pistol or shotgun (party members seldom use shotguns properly like a player would).
Mordin is quite good actually if you max his incinerate and give him a tempest smg. Incinerate cuts through armor fast and his SMG will cut nicely into shields, health and barriers. Has better shields than most members if you level up his salarian scientist skill.
Zaeed is very very good with maxed incendiary grenade. That skill does more armor damage than any other skill I've seen yet.. even more than a maxed warp. Give him an assault rifle to cut through shields and his grenades will make short work of the armor right after.

Miranda is useful in any situation with Overload and Warp. Her squad health bonus is also nice, and Slam is a pretty cool power.

They're both good at taking down shields AND armor, although lacking in biotics but the double explosive rounds are pretty cool.
Plus biotics are pointless till the armor and shield is gone anyway, and by that time it doesn't really matter, tech powers are just as good.

For my insanity playthrough with Sentinel class Miranda is very useful with the warp and overload. Need those a lot. And Garrus with sniper rifle and overload as well.
But really, it depends on the mission and also on the class you play as.
In ME2 unlike ME you have to switch between party members to get the most out of it.

ya, its completely situation. but, i will admit that playing my insanity soldier, i had surprising success in the collector base with samara and legion on my squad the entire time.

I found Samara to be the best party member for my Sentinel Shepard.
She has Reave to deal with barriers and armor. With my Sentinel, I loved having a character with Pull around in order to do the Pull/Warp combo, which is ridiculously powerful. I imagine this works less on higher difficulties, but on veteran, a character with Throw is great for Husk killing. Lastly, all NPCs have higher cooldown times, so they use their weapons more. Samara can use assault rifles, which is great.
It really DOES depend on the situation and the class of your Shepard though. Samara wouldnt have been nearly as great, I think, if my Shepard didn't have Warp to combo with her Pull.
Also, one should use a different loadout depending on what youll be facing. For instance, if you are going against Geth, then Legion + Tali is really nice for dual hacking. Zaaed is great for Disruptor Ammo in that case as well. If you are doing the final mission, you'll need to take down armor and barriers a lot. Samara's Reave is awesome for that. I could go on.
Party members are also good to plug holes in your Shepard's abilities. For instance, if you have an Adept, you will have trouble taking down shields. Zaeed will suddenly become really great for the Disruptor Ammo.
In the end, I think Bioware did a really good job of making all the NPC's useful. Personally, I used them all a fair bit.

On insanity everything has armor or shilelds, ever husks, so Jack/Samara = kinda useless. I use me (sentinel), Miranda, and Thane for a triple warp, double overload party.
