For pure combat Miranda and Samara tear it up. (mostly because Warp is so effin powerful in ME2).

For my Engineer: Thane, Garrus, and Miranda, I switch around regularly. But everyone is useful.

Actually yeah, I stick by the fact that it's situational, but if I had to pick a squaddie that is useful in the largest number of situations it would definitely be Miranda.
Miranda for me. She's just so useful. Another reason why I like her so much.
Her Overload is great against shields, Warp inflicts massive damage to barriers/armor/health (it's very effective against Harbinger), Slam is a really cool biotic piledriver and her class bonus Cerberus Tactician grants extra health to every squad member.
Awesome partner for my Vanguard. Never leave the Normandy without her.

Thane/Garrus combo is great. Miranda/anyone combo works well too. Legion/Grunt if I know scions will be around. Zaeed/Mordin vs. heavy armor types. Tali/Legion when I feel like sniping (double drones to keep them busy while I line up my shots).

Very difficult to answer. But as a soldier on hardcore Tali and Legion are great for breaking down shields and combat drones to keep the heat off of you.

Miranda is incredibly useful, and definitely #1 for me. She gives passive bonuses to squad health and weapon damage. Warp and Overload means she is effective against all enemy defenses. And Slam is basically a one hit-kill against an unguarded enemy. There's no situation where she's not useful.
Grunt is probably #2 for me. And Jack for any situation involving lots of melee threats. She's got the shotgun for close range and Shockwave to take them out in clusters.
I haven't played vanguard yet, but once I do I imagine the sniper characters will seem a lot more useful - to cover me while I get in close.

My default team ended up being Tali & Jack. If there was a substitution, it usually was for personality/plot/variety reasons.
I was a soldier and I had the same. Tali and Jack (Jack was a LI), I played on Veteran and it felt like I was on easy.

The first time though my favorites were Tali and Miranda. Playing now on Insanity, so far it's Miranda and Zaeed. Zaeed (and his inferno grenade...) is almost invaluable now with so much armor on enemies. Whereas Miranda's just too valuable to sideline for her squad bonuses and ability to warp and overload.
Now when I face synthetics, I'll still go with Tali and Legion. Two ai hacks and two drones is just too good.

Thane was a *REALLY* good shot, so I usually took him and someone with mostly Biotic abilities. During the suicide mission, he and Samara were my constant companions. They work well together, with their dual Throws and everything. 
Even though Mordin is super squishy, he could take out a line of dudes with his Incinerate. It was amazing how powerful he was, while also being soooo fragile. I used him a lot, as well.
I really enjoyed Legion and Tali. My first playthrough was as an Engineer, and during Legion's loyalty quest I took Tali. It was a cake walk. With that many AI Hacks, getting through that place was like a hot knife through butter.

Strictly combat I liked Jack. When she hit's that shockwave just right it's like skeet shooting, and it's effective for knocking weaker enemies from behind cover so they can be quickly killed, and you can focus on the big guns.

I dunno about everyone else, but Pull + Concussive Shot is epic. Launching enemies across the room never gets old. Garrus + Samara FTW :]
Miranda will always be a very good for Shepard though, the extra weapon team weapon damage turns into a lot for an Infiltrator.

Thane and Garrus complemented my Vanguard pretty well during my first playthrough. Miranda's an amazing character for weakening the opposition, though. Garrus and Thane have Overload and Warp, respectively, and both add an offensive power to the mix. Which is pretty awesome.
