None, I'm anal and did all the loyalty missions and all the upgrades.... and I put whoever they suggested in the role they suggested during the suicide mission.

Tali and Jack died before I even reached the Collector Base. I was heartbroken but at the same time I thought it was amazingly cool. Then I lost Legion died after the tube, Garrus got pulled away by the swarm (more heartbrake) Samara and Grunt died in the end. Don't remember if anybody else died. I loved it even though it was sad. I think this is really one of the game's selling points.

Jack, who was not loyal (lost her loyalty when she had that fight with Miranda and I never gained it back). She annoyed me so much with her off-putting personality that I chose her as one of my squadmates in the final battle in the hopes that she would get annihilated. She did.

First playthrough no dead squadmates but lost half the crew due to the fact that I messed up with doing an extra mission. Really shocked when I got to the tubes and Kelly dissolved in front me. And really sad when I got back on board the ship and I was seeing all the missing faces. But that is my 'canon' playthrough since it was based off the first ME1 playthrough I did without knowing how the game works.

After reading some of the other experiences people had, I have to mention that I also made sure that all of my team were upgraded to their level 4 specialist evolutions and if they had special weapons and stuff I researched them e.g. Jack's special amps.
The conversation you can have with Kelly on the ship after the mission is over is hilarious!

I lost half. Legion who got a rocket through his head, Jacob who was taken by the swarm, Grunt who was hit in his chest, Zaeed who died protecting the crewmembers, Jack (dont care about her) and Mordin just got killed when you get back to the Normandy. those coffins made me really sad : (

I have to admit I was surprised that everyone lived on my play-through.
I played my cards according to the storyline,
Tali being the tech savvy lady was put in the vents
Garrus made a squad of his own on Omega and it did great up until he was betrayed so I figured he was the best candidate to lead the second squad both times.He did get shot closing the door which got me all "O NOES" but then he was like just kidding and walked it off.
Thane's watchdog pursuit mission made him ideal to watch over the remaining crew on their escape.
Samara is an all powerful biotic so that just made sense to use her...
I ran with Grunt and Tali in the end and we all made it back happily ever after, Tali even gave me a hand back up into the ship.

Old Flashlight head bit the dust in my 1st play through. I was happy it ended up that way. Oh I also lost half the crew as well which I wasn't happy about.

No one died...at first.
First section:
Tali in vents, Jacob leading the fire team, Garrus and Legion on my team. Everyone survived.
Second section:
Jack held the barrier, Miranda leading the distraction team, Grunt escorting the crew back to the ship, Garrus and Thane on my team.Everyone survived.
Final section:
Garrus and Samara on my team(Garrus is my man). Mordin died on the way back somehow, and this is where it kind of fell apart.
I restarted the mission(I really wanted to have everyone survive on my first playthrough), because I'd heard somewhere that whether you lost someone on the way back was random. Nope; did everything exactly the same; lost Mordin again. Restarted again, replaced Samara with Mordin.
Tali died, and she was my love interest.
So I checked on, well, this forum for a possible solution, and I found that whoever you DID NOT take with you on the final section matters. So long as you leave Grunt, Garrus or Zaeed behind on the final section, then no one will die on the way back to the Normandy. I hadn't downloaded Zaeed, had sent Grunt back to the ship with the crew, and had taken Garrus with me on the final section, which was why someone kept on dying.
So, I did everything the same as I had the first time, except I sent Mordin back to the ship instead of Grunt, and guess what? Grunt was good enough to not get anyone killed!
So, in conclusion, no one died.

I lost Legion in the vents because I had Zaeed lead the firesquad.
I lost Grunt because...the Kool-aid man is red, I guess. (He was shot in the stomach)

First playthrough only Miranda died. I picked Jack over her in the argument and lost her loyalty and I never was able to gain enough paragon/renegade points to convince to her to become loyal again. It was quite disappointing because I really like Miranda's character.
On a side note, on my second playthough I was the ultimate Paragon, everyone loyal and everyone survived. FTW

I lost Garrus--he has apparently lost the ability to eat rockets and survive by the end of the game--and Jack who I figured would make one hell of a distraction leading the distraction team. I didn't really expect her to survive--a heroic death is better than a life of psychotic criminality.

I lost Jacob in the vents. I naively believed he was up to the task. Mordin died in the 'hold the line' phase.

Oh I feel bad for laughing at this.
Anyway, on my first playthrough I lost Miranda who was not loyal. Second, no one died since I learned my lesson and went full paragon. Not convinced, there will be a small-scale massacre on my current playthrough. Unfortunately because I like all the characters and it will be hard.
