I almost want to mess with the loyalties and try everyone in team leader 2 position to see what everyone's last words are... That may be a little sadistic of me.

I didn't have anyone die. I read in some other threads what some people did for the final mission and I didn't do exactly what some other people did. I think it boils down more to don't pick the "wrong" person for the job. I thought most of it was common sense. There's also the possibility that you didn't do enough research for better weapons/armor and the like for the squadmates. I did all the research I possibly could.

Then Zaed leading the distraction team. The shields didn't hold he said, took a bullet to the gut.
Then Mordin when they left the damn place, not sure how the heck it happened, he just lay there dead.
All crew survived.

I picked Garrus as a team leader (since he definatly has experience, and legion to scout the tunnels, since he is experienced and tech and well, if anyone wants to face suicide it must as well be a robot.
Everyone lived.
Then I picked Thane as a biotic and Mordin as team leader just to mix things up.
Baaaad mistake, I felt so bad seeing Garrus disintergrate before me, as well as Mordin's "I held the line..." as he died, that I reloaded and got Samara as a biotic and Garrus leading again. Garrus is so baddass the bullets didn't slow him down. I sent Zaaed to escort the crew because I wanted to get him out of my hair and he did fine.
So I took Legion and Grunt with me for the final battle, knowing they couldn't die, and if they did, it wouldn't hurt me as much....
And then one of my if not my favourite Mordin died.
Sure everyone else lived but who will cure the genophage now?
I'm going to have to reload but I'm still trying to figure out what I did wrong. Maybe take him with me... or send him to escort the crew.
Maybe he just wants to die....
I don't know if I should reload... what did I do wrong? I had all upgrades and all my guys were loyal.
Still amazingly... emotionally engaging having someone die, despite my efforts, and watching the coffin, knowing it was for the greater cause.
To those who had everyone survive, who did you take for the final confrontation. I think that must have an effect on who dies...

I picked Legion to go through the vent with Zaeed leading the other team. Then picked Samara to lead the second time (thinking I needed to pick the biotic to shield them).
I was worried for a second about Garrus and Tali (my team) after the finale fight, but they survived.
Even so, the Normandy is a very empty place without half of the crew... and without Kelly, how else am I meant to know when I have unread messages?
Even so, I loved the ending to this game.

garrus (natural leader, mentions leading a squad bevore) for the teams
legion through the vents
samara for the barrier
and jacob for the rescue
jack and grunt where in my team.
also got all the upgrades i could find
also the ending was....i could say moving.
i havent seen a game ending like this

in a conversation with mordin, he makes reference to captain kirrahe, the salarian leader who gave the infamous 'hold the line' speech. mordin says something to the effect of 'his speech was inspiriing, and good for morale; however, i do not like holding the line. i prefer going, not doing'.
asking mordin to hold the line is like asking garrus to hack the door. they can, but they aren't so good at it, and in mordin's case, he flat out dislikes it. escorting a crew is a special ops type task, something mordin has experience with.
your best bet is to have him escort the crew, something in hsi area of expertise, and not have him hold the line, something he is not adept at.
note: i had him hold the line for me, and he survived. i attribute to this to:
1: picked the same leader for both teams (garrus). as any good leader will do, they will notice who is lackign at what. after seeing mordin not do so well the first time, he was able to give him extra cover/support the second time so that he wouldn't get hurt.
2. plain old good luck. everyone was loyal, so that may help.
once more for reference:
tech specialist - legion
1st team leader - garrus
biotic specialist - jack
2nd team leader - garrus
personal squad for all fights - tali and samara
escort - zaeed

I chose Garrus for all the other team leaders, I had Legion do the hacking and then Miranda hold up the biotic shield. And Mordin take the crew back. All survived except Thane who for some unexplained reason wouldn't run back to the door when called and got hauled off and devoured by the swarm.
This was intolerable. So I reloaded and didn't bring Thane with me and took Samara instead of Miranda to hold up the shield. No being tardy AND at the end Samara makes this great big biotic explosion to throw a few collecters away. Now no losses at all.

saving the crew:
they live depending on how many missions you do after the collectors take them.
*note - i am not sure if assignments count as missions in this case. your best bet is to assume that they do.
1 - everyone survives.
2 - the engineers, kelly, and half the crew die.
3 + - everyone but dr. chakwas dies
now, for the actual position assignments:
tech specialist - only tali and legion can successfully do this without dying. however...
1st team leader - garrus, jacob, miranda
.. if the team leader is not one of these people, then the tech specialist will die no matter what.
biotic specialist - jack, samara
2nd team leader - garrus, jacob, miranda
remember: jack is considered the strongest human biotic, and samara is a justicar. only they will be strong enough to keep everyone safe, otherwise, one of your current squad members dies at random.
escort - anyone will work. if they are loyal, it's a ticket to survival. if they are not, they will die. mordin is probably best at this.
final battle - as long as both of your squad members are loyal, they live. whoever is not loyal will die due to falling rubble.
and for those with morbid curiosity:
how to kill everyone - do everything deliberately wrong and have no one loyal to you. if even one person is loyal and you don't have them killed before shepard begins running back to the ship, i believe there is a chance that they will be able to jump to the ship and pull shepard up. otherwise, if it's just shepard, he'll die as joker won't be able to pull him up in time.
