Which party members died in your first playthrough?

Tali - she hacked the door into the Collector base after complaining how hot she was in the tunnel. As she was helping close the outer doors she took an energy ball to her faceplate.
Thane - Killed by the flying bugs after everybody else got to safety.
Zaeed - Killed covering the surviving crew back to the Normandy.
Miranda - Killled leading the covering the door while Shep and pals went after the final baddie.

At the very end, I took her and Thane to the final battle, assuming she'd be more likely to die offscreen, and that I'd be given the chance to protect her. Also, I need all the warps I can get (I too have biotics).
In the end, I saved her from falling off of the platform, but she died underneath a piece of random debris. Thane, and everybody else, made it.
Anyone know what happens if both companions die, but everyone else live? Curious.

First (explanation here)
1) Samara was picked as the leader and Miranda approved. Legion (figured was the best option) got rocket in his face when closing the doors.
2) Now that I lost a fully loyal, upgraded member I did not want to lose anyone "important". I figured the cerebus guy is expendable, so I chose him as the biotic. Instead of dying, Jacob got saved by Thane who was killed by the swarm shortly after. Samara, still leading was shot in the stomack and died... so much for a millenia of experience.
I found out that Garrus is suppose to lead and completed with 0 casualties. Tried with Zaaeed and Grunt (can't regenerate after being shot, seriosly, supreme, pure Krogan??) as leaders, they both ended up dying.
Had Mordin and Thane with me, I find them most useful as a charging vanguard on insanity. One to take out barriers/armor the other for armor AoE. They made it both times.

So I reloaded, upgraded everything and killed the boss in record time and everyone lived.

Any remaining squad member will pop out of nowhere (it's obviously the door, but it doesn't show them coming out) and pull you up.
Anyways, for my first playthrough, since Miranda said she could do it, I used her for the biotic field and took Garrus and Tali with me. When Miranda fails, Garrus gets taken by the seekers, I thought Shepard would save him for a moment, but he was taken away. I was very sad, but I pressed on and lost Mordin while he was defending. I saved all the crew also.

2nd Team leader: Jacob
My squad: Zaeed and Jack
Escort: Tali
Biotic: Samara
Distraction squad leader: Miranda
My squad: Legion and Jack
Final confrontation squad: Grunt and Samara
Might need to reload and have another run at it.

Went back through with a SLIGHTLY different setup someone posted and she lived. The setup was exactly the same, except for the person who escorted the human survivors back (Jacob, if you're interested) and I had different squadmates come with me. I honestly doubt Jacob would have influenced that life/death. How does loyalty effect a rock falling onto you? Karma I'm guessing? Oh well, I have both saves with my original saving the Collector base and my second run destroying it.
edit: If you want to save a disloyal Jack, here is the setup
Vents: Tali
Second squad leader: Garrus
Biotic sheld: Samara
Escort humans: Jacob
Second squad leader again: Garrus
Final squad heading into reaper fight: Garrus/Tali
I am not sure if initial/final squad choice plays into it. I think I had Grunt and someone else to start off, I copied someone's setup word for word on my second time through. Again, the only major difference was Jacob being the escort. Otherwise my first playthrough was exactly the same as this one.

Am I the only person that thinks its unfair how if the Biotic barrier person fails, someone else suffers? Especially when it's Miranda who basically says 'anyone can do it'. I mean, it doesn't actually affect me cos I used Samara (I was a bit worried when she got really exhausted and looked like she'd collapse)

I lost Jack and Thane right off the bat. They weren't loyal, and I didn't have all the ship upgrades.
Lost Garrus to a swarm of bugs while trying to get through a door. He was loyal. Must've put him in the wrong team or something.

second time I intentionally got Zaeed killed by sending him into the vents..there needed to be atleast one death, and I disliked him... >.>

I completed my first playthrough exactly how I wanted to. I used my main ME1 Shepard, followed a mostly paragon but occasionally renegade path, completed every side mission (and DLC), kept the entire squad alive, betrothed myself to Tali and told Illusive Man where to go.

If you have a disloyal Jack, send her to escort the colonists. I've heard that's a "get out of the mission alive free" card. Haven't tried it myself (my paragon and renegade managed to pacify her and Miranda) but I've heard it works for some people.

Second playthrough I lost no one though!
