Mordin died along with Kelly, Gabby, Rupert and other Normandy crew members.
Fair play to Bioware for killing of the crew. I avoided spoilers on my first playthrough and didn't know that the aftermath of the IFF was happening in realtime.
I call BS on the mysterious death of Mordin, though. I reloaded and brought him with me for the boss fight, then sat on him Private Ryan style.

If I remember right..
I lost the crew. Chakwas didn't have an escort and died. Garrus, Zaeed, Miranda and Samara. Garrus and Samara had particularly cheap deaths.
I kind of wanna do an intentional bloodbath playthrough at one point

Amasingly, none. I had all of them except Jack loyal and fully upgraded. I diden't had enough paragon / renegade points to aleviate the conflict between jack and miranda, and I choosen to side with miranda. Jack wasn't given any specific mission during the Collector's base mission, he was part of my squad for 1st part. Everyone made it through, diden't loose any crew members either. I did not go straight through Omega4 relay, I first headed off to Omega to purchase last upgrades as I did not have the money for them earlier. Last part of mission I did with Miranda and Thane in my squad, myself playing a female Engineer Shepard. The whole suicidal mission felt more easy than other game parts for some reason. Maybe due to my squad choice this time, or being higher level / upgraded perhaps.

I had Samara hold off the swarmers.
And I had Grunt and Tali with me the whole time. (after Tali went through the tube)

Didn't notice that but wierd non the less. Anyway, I was thinking and I also want to ask if it matters if we upgrade our team completely or not. I may have missed a few things and that may have contributed to team deaths. I also tried again and Lost legion in the tube and Grunt inthe diversion team. Lost people in the same places as before but different people.

I lost Thane. He led the distraction Party while I went through the swarm. Thought he would be able to take care of anyone. Who better to cause a distraction then an assassin, who is a master at small arms. Well I learned my lesson.

I lost legion. Almost thought I would lose Miranda as well as she got shot near the end, but she pulled through
Was a bit worried about Tali as well after removing the debries field, but she was ok
On my second playthrough now with a renegade Shepard. Lets see how that pans out.

Didn't lose anyone (finished the suicide mission twice). Paid attention to alternate squad requirements (Tech, Leader, etc) and had everyone loyal.

I can't buy that he's not one of the more qualified, c'mon, he was supposedly a master of infiltration.
On my 2nd playthrough I had THREE die, lol. Garrus, who I sent through the ducts, Zaeed, who I made the "cover our back door" team leader. Thane ALSO died, he got ate by bugs, and I didn't understand that one at all

I must have got lucky. I didn't lose anyone as far as I know and I rescued all the crew as well.
Tali was my tech.
Garreth was my second squad leader. He was really good at it actually.
I kept Samara and Mordin with me. Though she was my highly trained biotic of choice for the swarms.
Kept the Grunt with any other team in the hopes of them having someone tough.
Thane was used to bring the crew back to the ship, because I figured him to be sneaky.
Tali was my tech.

Just Mordin, stupid squishy Salarian. I redid it with him leading the group back to the Normandy and no deaths that time, had a few people with the same prob, full loyalty and upgrades, just seems like random chance.

The first time through I lost two people, and I liked that ending more than the 'everyone lives' version, far more emotional and personal for me.
Tali for the tech, and Garrus for team 1, no problem.

Grunt, Samara and Jack. Jack was a given because she was the only team member not loyal (Paragon/Renegade weren't high enough to stop the argument between her and Miranda).
I reloaded a save and tried the whole thing again, and that time only Jack died, but on a different part of the mission than the first time. The second time, I was afraid Garrus would die, because he got shot in the same cutscene Grunt had been killed in the first time, but the shot apparently bounced off Garrus' armor.

I lost Legion and Zaeed. Everyone was loyal to me and I'd upgraded everything. Seems I picked the wrong person for certain jobs. Well, better luck next time I guess.

Legion went into the tunnels as the hacking/tech expert.
Miranda lead both fire teams. Samara did the barrier.
Tali and Jack followed my sentinel Shep throughout the entire base fight.
It just felt right having Tali since that was my LI. In retrospect I made that entire ordeal much harder than it should've been.
Grunt escorted the survivors back.
When it came time to decide the base's fate, I remembered Shepards quote from ME1, on Virmire. "I'm going to blow this place to hell and gone."
Which I did. Total paragon the -whole- game, with everyones loyalty.
As a side note I have to say it was a laugh out loud/cheering moment when the Normandy destroyed that collector ship. Jokers "Sons of ****es" comment was priceless.

I managed to get my whole team through. Legion was my tech expert, and Garrus lead the secondary team while I moved in with Tali and Miranda. Although Miranda got shot as our teams re-converged and I was worried she was going to die, she pulled through.
Zaeed went home with the survivors, while Jack provided biotic escort for Shepard, Tali and Thane and Miranda lead the second squad.
Finally, Shepard, Tali and Miranda went through and put rest to Harbinger's plans despite his insistence that we had done nothing. It was nice to see Miranda ignore the Illusive man, and cut him off.
But my real panic came when I thought Tali was going to die falling off the big platform... and the small worry that she or Miranda might have been totalled by giant girders.
It's time for this team of galactic fighters to take down the reapers.

i keep losing Legion when he tries tp close the doors in the collectors base on the final mission. This is my second run through and both times i have managed to save all my team mates, but i lose Legion in the same place. On this run through i have done all the sub plots and 100% of the galaxy, I use Legion for the vents, Zadeed for second team, samara for the biotic shield, then i take Thane and Jack with me on the last leg of the mission....what am i doing wrong

I just finished the game yesterday and all of my squad members survived the mission and so did all of the Normandy crew which was a pleasant surprise for me. I checked after the game was over by walking around the ship and seeing all their smiling faces.
I made sure that I did all the loyalty missions before going for the final mission. That's just the way I play. I had no idea what would happen because I purposefully stayed away from these boards and any spoiler info.
When they asked me for choices on squad members, I got very worried, because I remember the Kaiden/Ashley thing from ME1 and thought, damn they are going to die! I am glad nobody did!
For the first firesquad team, I put Garrus in charge, as they said they wanted experience and he also had no negative issues with the rest of the team - plus he was Arcangel and had his own team on Omega. So I thought the leadership was good for him. I was right.
I sent Legion through the vents, because I was asked for the best tech and a Geth seemed to be the best choice. He made it through OK once I opened all the blocks.
I picked Jack and Miranda for my own squad members every time - I liked that they had issues with each other (which I dealt with Paragon style) so I wanted to keep an eye on them. Plus I needed their mix of tech and biotics, as I am the default soldier class with no biotic or tech powers. Also Miranda was the love interest I chose so it was also cool to have her by my side (it made the whole falling off the platform moment and grabbing Miranda's arm just in time much more dramatic!)
For the second firesquad I picked Garrus again and he got everyone through alive. I picked Samara for the biotic shield because she is powerful, calm and controlled. Plus I already had Jack on my squad as I mentioned.
After saving Kelly and Dr. Chakwas and the rest of the Normandy crew, I picked Jacob to escort them back to the ship and he survived and got everyone back with no casualties! I thought he had the skills and was right (loyal, mix of tech and biotics and secret handshake best buds with Shepard).
It's good to know that as I was aiming for all the right choices first playthrough I got them all - it made the ending much more intense and thrilling for me! I loved this game!
