Thane distinguishes the actions of the body from that of the soul. In this regard, his years as a hired gun leave him spotless... don't they? He tells Shepard that what made the Batarian deaths different is that it was his conscious decision to hunt them down and make them suffer. However -
1.) Technically, wouldn't their attack on his family be considered sufficient enough for his reaction to be seen as self-defense? Or at least in defense of his son? Ok, maybe not, especially since he admits to... dragging it out. And these guys were the "trigger men" ... like he is. They were hired. Like he was.
2.) Assuming he is telling the truth when he says their deaths were the *one* time he acted on his own, why does he keep referring to the evils he's done, the things he's taken from the galaxy, and to himself as the wicked? Killing Nassana, well, she was a contract. He should be guilt free there, but he's praying for himself. And how long does it take to atone for murdering a bunch of Batarians anyway?
The evils he is referring to are more to do with torturing those batarians. He says he took his time with them. Usually he kills instantly. I think that the torture part is more problematic for him and also the fact that it was his decision to kill them. And he also feels guilty for abandoning his family, he was the reason for his wife getting killed and Kolyat not having parents. Those are the evils – I think.

edit: played through his recruitment mission again. First conversation after has both the life expectancy in it (if you investigate) as well as the hanar genetic engineering program which he says he doesn't believe he'll live long enough to benefit from, and the option to ask if there's anything they can do for him on the Normandy (v. noting that he seems calm. You can only pick one or the other, and both will end the conversation).
He has for who knows how long to live. Because he says that he will be fine for another 8-12 months. After that he will not be able to be in your crew but how long he will live after that is not sure. The death is supposed to be slow. And I also do not think he looked up for any treatment. There aren’t any cures but even in the codex it is stated that a lung transplantation can buy time - he didn’t bother with that either. And that he doesn’t believe he’ll live long enough for the treatment is just a pessimist in him talking. He was probably just waiting to get back to his wife as soon as possible.
But the question is also in what time frame does me2 take. If it takes you a year to do all those missions then they will need to treat him somehow if he is going to be in your crew. But in any case he won’t be dead in me3 like everyone else who isn’t his fan assumes.
But from the lung transplantation, chain letter, breathing masks, something fishy about hanar, the fact that this is a sci-fi and not a drama, and now this omni-gel for lungs - I think he will be fine or Bioware is made of bunch of mean bas*****.
