Aw, I don't know about that... I get why he'd be reticent and not think of her romantically in the first two games, seeing her with another guy... I do think Joker should only be an option if the player ends any other active relationships they have, but other than that, why punish forever the Shepards who previously tried to content themselves with the kind of guy they can sleep with? I have a single character, but my Thane-loving Shepard who didn't romance Kaidan might be pretty damned lonely if Thane dies of Kepral's syndrome.
Honestly, loving someone who's dying is easier than loving someone you think you have forever with. You're going to put your selfish impulses aside a lot more readily. And after it's over, you're going to be lonely--who better to try something with than somebody you've known a long time, somebody you trust and know will be there? But any Shepard could easily make a mistake of the heart, too... fall for someone she's not suited for, or fall for someone she can't keep, and then be left to sort out how wrong she was. And then in the process of that, maybe she realizes the guy who's always been at her side is pretty damned awesome.
The nice thing is, who is like Joker in the game? Nobody! So loving a second time is definitely not going to be one of those sublimation things. You don't go after one of your best longtime friends and crewmates unless you're pretty freakin' sure you're not gonna screw it up. And maybe that, more than anything else, explains the current vibe between Joker and Shepard--neither of them is sure it could work, especially not while he's still got that bone problem, so they keep everything platonic because it would only screw things up if they tried it right now.

For that matter, I'd like to set Dr. Chakwas up with something... some kind of medal and public recognition for all she's done in your service. I want her to find a nice younger man to hook up with. men are the shizznit, as the awesome as all heck Sar above said, it is
truly hard to be sour when you've got funnies being thrown at you left
and right. Relationships are at their best when people aren't going
around happy-humping each other or staring luridly into each other's
eyes whilst spouting romantic nonsense, but when you can make each
other laugh. Those are always the best!
