I've thought about this, too. I totally jaw-dropped and thought "Did he seriously just say that?!" when he said "That's what you get for dyin' on me".
I think they're both the type to suffer in silence with their personal demons. Joker is obviously uncomfortable with being "touchy feely", as referenced when he told the Commander that Chambers is good for that sort of thing, but that he is there for Shepard -- only to ruin what could have been a caring moment by adding "ya know, up here with my things". He just has to diffuse any serious talk with joking. It's his defense mechanism.
Shepard seems to put the focus on everyone else but herself (or himself), always asking the crew if they need anything and going out of her way to help them. Like, seriously guys, get over yourselves and do the job you signed up for. (I'm not really complaining about that, I love the loyalty missions

). As far as dying from saving Joker goes, while it would be in her nature to ask him if he's okay it would involve dealing with her feelings about it as well, and since she doesn't burden her crew with her own crap I think she would avoid it. I think she would just let it be.
Perhaps I've overthought it too much, lol... but it's something that could be taken into account for progression of their friendship and potential for developing a romance in the third. Or on the renegade side, make their relationship strictly work related; cold, distant, and to the point, though that seems extreme and highly unlikely since Joker (aside from Chakwas) is undoubtedly the most loyal of all the crew. Even if Joker isn't eventually romanceable, at the very least it would be great if the whole "you died saving me/I died saving you" thing would be addressed.
