In the mission from Ish (omega), you can collect a package from either Marab's (Citadel) or from Eternity (Illium). I got a mission update from the Citadel datapad, and went back to Ish in Omega. Having a very high Paragon rating, I was able to convince Ish to abandon his scheme, which updated the mission, but has still left it open (even the mission title description states I have delivered the packages successfully)

It's been reported several times... there's a good thread about it which I make a point to bump every day. Hopefully they fix it...
