I did some experimenting with the final mission and difficuly levels.  I'd say the difficulty level might be that X Factor.  It seems the higher the difficuly level, the more likely someone can die.  I know it sounds like stupid common sense but here's what I've found.

On Casual difficulty, if everyone is loyal, the Normandy is upgraded, and everyone is selected for their proper job, everyone will survive it 99 percent of the time.  There's probably still that random chance someone can die, but I've done several Casual playthroughs and everyone seems to live every time.

On Normal, you need to have everyone upgraded on top of  being loyal, having their proper job, and having the ship upgraded.  You can get away with not having them all upgraded, but the likelyhood of death is greater.

On Veteran, you need everything listed above, plus you need to pick the right people with leadership skills to lead the other teams.  On Veteran, I think the game begins to factor in that stuff and who you have on the fire teams affects who else might die.

On Insanity you need to pretty much be perfect.  All upgrades for the ship and team members, all loyalty, and all the proper jobs assigned.  You need to pay attention to team leaders and make sure the team holding the door has the proper leadership as well.

This is just based on several playthroughs of the final mission on all the different difficulties.  If anyone can refute any of this or elaborate or add anything please do so.  It seems like every playthrough really was made to be unique and different.  Kudos to BioWare, because here we all are trying to figure it out.
