I finally found a way to make it work.
1) Go to Documents\\Bioware\\Dragon Age\\
2) CUT your Add Ins Folder and Paste it "somewhere else". Doesn't matter where, for now so long as you remember it's location.
3) Start Dragon Age, log out of your account. I exited Dragon Age and restarted again, but I'm not sure that's necessary. Log back into your account and then exit the game.
4) Go to the location you pasted your original Add Ins folder. COPY (to ensure that you maintain a copy, just in case) and paste it back into \\Documents\\Bioware\\Dragon Age\\. It will need to overwrite the new (and empty) Add Ins Folder that the game recreated when you launched it in the last step.
5) Restart the game, resume your game, and the issues with Warden's Keep (quest NPC not appearing or not allowing you to accept quest) should be gone.
This MAY fix the issues with Stone Prisoner, too...I'm not sure as that quest line worked just fine for me.

I've been digging through these forums for days trying to find a solution for the fact that neither my Warden's Keep nor my Stone Prisoner quests were working. I applied your fix and now both work perfectly. Thanks so much for sharing your fix.
This post needs to be either be made sticky or the info needs to be added to the DLC troubleshooting faq.

Just wanted to say thank you! I've been searching for this fix for a few days now and after some wrangling it worked!
My fix was a tad different and thought I'd post it here though.
I followed steps 1-3 of the original post.
After logging into the game -without- the Add Ins folder, I allowed the game to download all the DLC again.
After that, everything worked fine, and my original Add Ins folder is still sitting on my desktop collecting dust. I'm not going to delete it yet, just in case something went screwy (all I've done is start the game and see if I have the Warden's Keep quest yet, which I do now).
Hopefully this fix helps everyone perplexed by their DLC not showing up!
