Given how fast it happens, I'm not surprised if many didn't even notice.

I've never had any problems with any of the interupts. I don't understand why people complain they appear too quickly. They're up for at least 3 seconds.

i missed a couple until i got a little experience with them. they could leave them up a bit longer at first.

If I had a save close to where I was I would always go back and re-watch, re-do the scene without using the action.. I thought the most interesting were the options right before a gun battle.. to see who would actually start the fight if you didn't pull the trigger.
