Do you have to send Mordin back with the crew?

The first time I tried to get him to do the tech expert thing, he died closing the doors. The second time I didn't have him do anything, and he died in the final part while everyone except my squad was holding the door.

2. Sent Legion in the tubes, Garrus first fire team. Took Miranda and Tali with me
3. Samarra for biotic shield, Miranda led second fire team. Took Grunt and Tali with me
4. Thane went back with survivors
5. Took Tali and Miranda for final fight.
No one died.

Regardless of your specialty role choices, in order for you to save everyone (apart from having loyalty across the board) your second team leaders *MUST* be chosen from these three:
Jacob, Miranda, Garrus.
It doesn't matter in what combination or order they are placed. You can even have any one of those three be a Team2 leader twice in a row. However, puting anyone else in a leadership role will cause inevitable deaths, regardless of loyalty and full squad equipment upgrades. If anyone else has been -completely- successful using any other Team2 leaders and escorts, feel free to add to that list.
Addendum:
Took out the part about the escort. Indeed, it seems that all it takes to protect the crew on their way back to the ship is any loyal party member.

I honestly think Mordin can just die randomly.
^ I don't know if the escort matters. I sent legion back and no one died.

I had Garrus for team leader both times, Justicar for biotic
Legion as tech and Zaeed for team escort. Took grunt and Garrus to face the reaper.

No offense, but you must be doing something a little off. According to the mewiki, it goes like this:
Techie - Legion and Tali live
Biotic - Samara/Morinth and Jack live
The fireteam leaders must be Jacob, Miranda or Garrus, with loyalty.
The person protecting the crew must have loyalty, other than that it can be anyone.
I followed this guide and everyone got through all right.

I have to say I was totally surprised when people made it sound like he was hard to keep alive. On my first playthrough I screwed up a few things. I kept Grunt and Legion in their respective cages out of paranoia, I sided with Miranda over Jack and never got her loyalty. In my zeal to get Jack killed I ended up losing 3 people in my squad, and none of them was Mordin (lost Jacob, Garrus and Jack). So I am very suspicious when people make it sound like its a real effort to keep Mordin alive. I finished the game 3 times so far and haven't lost him once.

I've let him shoot the other Salarian and deleted the data afterwards, he lived through the final mission. Maybe either of these somehow factors into it.

regardless of your listed choices Mordin still randomly dies , i reloaded a quick save to bring him with me and everyone else survived

The only reason I can think of why he did is because I took too long on the Reaper fight. My first game he survived and I stopped him from shooting. Second game he died and I let him shoot the guy.
I don't think that factors into it. Loyalty is loyalty. Its either some horrible bug or it has something to do with the length of the reaper fight.

I am not a big fan of shooters (ME is the only one I will play) and I am not particularly good at them. The way I configured the team was to make things easy for me. For my style Tali's combat drone is very effective at keeping enemies occupied (i.e. shooting the drone instead of shooting me). Plus with all the interviews and pre-game media releases, I learned that there was a chance of squad treachery. Tali to be honest was the one I trusted the most, so I kept her at my side throughout the final mission. I did not romance her yet, its a work in progress on my current playthrough.
One more thing, in reference to the mewiki post. Tali/Legion for the tech role does not garantee survival. I have heard of people having them die when assigned that role. From my personal experience, the Tech role survival has far more to do with who leads the first fire team.
