best way to deal with the brood mother is to take out each tentacle one by one then focus on her rinse and repeat for when she summons soldiers and for when the tentacles come back.
As for the Urn quest there are several ways to start it one is to simple run into one of the knights who are questing for it (which is the way I got it) or after you've dealt with the demon in redcliff castle Ban Teagan will tell you they need the ashes to cure the Arl

The tentacles come back? About how long do they take to respawn, because they're tough enough to take down as it is.

Here's a secret. Tentacles cannot come up through the rocks, if you line your party up on the back wall on the stone you can use ranged attacks to bring her down and just intercept the adds with your melee. Ignore the tentacles entirely like this.

I think its probably just once she gets down to a certain amount of hp they come back.
If you have too much trouble with it you could always lower the difficulty level a notch for that fight.

They don't respawn, really. if you damage one enough it retracts; once they all retract the broodmother is vulnerable. Do some damage to her, and they pop out of the ground again with the same level of health as they had when they went in, and some darkspawn join the fight. Hurt the tentacles enough again and they retract, and the cycle goes on. Once the tentatcles are dead (zero health), wail on the broodmother and you're golden. When the tentacles are out, don't even try to get near her.

I forgot to mention, when lining your group up on the stone space them out enough that her ranged attack doesn't splash to multiple party members. Her pitiful attack vs 1 person is quite easy to manage with potions or a mage heal.

Ranged attacks focused on the Broodmother are how I took her out, but i tend to micromanage my fights anyway because this is the RPG game I have really done, and I take it very personally when my characters bite the dust...
I had my PC Mage Fireball her, Shale use Hurl and Rock Barrage, and Zev bombard her from range with bombs and acid flasks when he could get free from the hurlocks.
Fortunately I also had Wynne to revive him when he got clocked.
I micromanaged, but I did take BroodMama out first time.

I never mess with the tentacles at all and I've always killed her in one try, even on nightmare. I put ranged fighters back on the stone entrance and my melee up in her grill (aka bewbie hell). When the darkspawn come out, the melee fighters go back to the entrance to help protect the ranged fighters then back over to broodmother. I kill her pretty quickly actually.

I hated the tentacles.... So I put my two-handed oriented warrior and Oghren in straight up Mele with the broodmother, never even bothered with the tentacles.
wynne had to run around in circles to keep healing everyone and survive the tentacles, they were out to get her!

When you take the tentacles out of the equation all you have to do it hit her with AoE spells if you have any and she will have no chance, after the tentacles have been permanently destroyed she takes damage very quickly.
Do not send your tank in to melee her as the she will end up grabbing him which is pretty much a one hit kill. Wait untill the tentacles are dead before taking it to her.
By the way, the tentacles come back at particular health intervals but after the third time they appear, (I think, mabey four), you can kill them permanently.
