Second time I came to this part in the game and the same pop in occured.
Once the romance scene plays out and then it suddenly shows the sun that The Illusive Man is always looking at, there is a random pop in of a man standing on the right side of the screen. (This is after being told to put in Disc 1 as well).
It happens for half a second. At first it looked like a real person, as if the devs put in a likeness of themselves all of a sudden, but it was so brief it could have been a mistaken pop-in of The Illusive Man from a previous scene with him on the same disc. Either way I have encountered this twice and thought it worth mentioning.
