Honestly, remember in ME1, when Ashley would call you "skipper?" Or Commander. Why not John? As you talk to her more and more, it becomes painful to see all of the effort Bioware had to put into avoiding your first name. This is only one situation, too. There are many sections of the story in which this is true.
For one thing, it totally undermines the feeling of building a rapport with any of your characters, much less your LI. Remember when you and soulmate are cruising around town and she just keeps referring to you by your last name? No you don't, because it is awkward and impersonal in that context. Same goes here.
I understand that making your own character and living your own story is the entire point of the game, and doing just that is probably the most enjoyable experience to be had in any current-gen RPG. However, consider the gains to be had in simply having a standard name for the male and female Shepards. No more awkward nicknames, weird workarounds, or just plain disappointing "Commander" from people you just went to hell and back with. I'm not saying that appearance or any other aspect of the characters need be restricted, but simply keeping a standard first name would work wonders to alleviate these concerns.

When someone is trying to really connect to you and make a good point, they will often use your first name. What I'm saying is that this would really improve the narrative framework and the impact of the conversations if all Shepards had the same first name. If naming is so important to everyone, we could make their middle names or something, or their nicknames. The first name would really help things be more credible though.

I have always hoped that at the end of Mass Effect 3, Shepherd goes absolutely crazy when he realises he is never called by his first name, and he then goes on a massive rant, and the credits roll in just as he is about to say his first name.

Because not everybody sticks with the default "John Shepard" name, especially people who've watched too much Stargate Atlantis and keep expect Wraith to pop up and start trying to conscript him into their own private missions.
To speak a name, it has to be voice-acted, but they can't get the voice-actors to speak every single name combination that is possible. What about foreign names? Chinese names? Silly names that people give their characters because they want to be space-elves? I think there may be some in-game written references to your first name (if not, there should be. If Baldurs Gate can do it, so can ME) but vocalisation of your name is impossible.
