I am playing on insanity atm, going for achiev.

I tried Legion (who was norm) for tech spec but he died, Grunt was escorting the entire crew to Normandy no deaths,  Miranda led first and will lead second fire teams, one groupmate dies on bionic shield, and I am sure both groupmates will die on Reaper.

I did loyalties for all but lost Legion and Jack loyalty due ot low Persuade.  Ship recieved all combat upgrades and group received *many* upgrades.....but not enough =(?

I got Mording his omni  tool, got maxed out Bionic, mostly upgraded Tech, tons of damage/armor/shield upgrades with some char specific upgrades.

Where did I go wrong?

My first playthorugh was on Vet, I didnt upgrade, lacked loyalties, and neglected good role choices to intentionally kill teammates.  I am getting the same results on this Insanity playhtrough.

At this rate I might just intentionally kill Jacob, Miranda, Jack and Samara since they are my least favorite groupmates.

Your specialists have to be loyal and the right picks for their jobs or people will die.

For one you probably need all loyalties to get everyone out alive. Next time through the game I suggest doing more side missions and miscellaneous quests to get the paragon/renegade points that will give you the persuasion options needed to keep everyone on your side.

Secondly, you absolutely need to select the right people for each job. The first concern you should have is if they are loyal or not. If not, don't use them.  Next, select the right people for the right job.  A good test of this is if anyone dies during their particular mission segment.  If so, then you didn't pick the right people.  Even the death of the tech/biotic specialist will tell you something about the quality of your fire-team leader, and any deaths suffered during the reaper battle probably have more to do with who you leave behind than who you take with you.

I recommend finishing the game you have now and restarting with these things in mind.  Also remember that you must have all relevant quests and side missions completed before you get the Reaper IFF.  If you wait too long before entering the Omega 4 relay your crew will be lost.

If you want some advice, my selections were Tali(tech team), Garrus(fire team), Samara was the biotic sheild generator and Legion escorted the civilians(an odd choice i know, but it worked). Everybody was Loyal except for Zaeed(i figured he's not nessicary since he's DLC) so i'm geussing Loyalty is pretty key to ensuring everyone survives.

Oh and i brought Miranda and Samara with me to fight the Reaper(i was fully techy so i thought their biotics would be usefull).

Go Tali for tech, Miranda both times for team leader (like you already are doing), Samara for biotic shield, and just to be safe, send Mordin back with the crew. Don't bring Legion or Jack to the final fight, and you will PROBABLY be good to go.
