A dev explain this somewhere..

Hold the Line is where you want to leave your best killers, especially if you want your squishy characters to live. So leave Grunt and Zaeed (atleast 1), Thane is a good shot.. Ah well you get the idea. And Mordin will survive just fine on the "line". =p

I never had a problem keeping everyone alive (squad+shipcrew), even in first playthrough without forumhelp.
Just in case someone wants to know, of course all where loyal.

Tali was Tech Expert
Fireteam was Jacob (both times when you can choose the leader of the "not your team")
Samara did the Biotec field to protect me
Grunt escorted the crew back to normandy
Thane+Legion in first, Thane+Garrus in second playthrough in my team

1st Team Leader: loyal Garrus (survived)

Tech Specialist: loyal Mordin (head blown off with a rpg)

With me: Jack & Tali.



I guess it's not a huge deal, but it's just annoying that i can't keep the specialist alive, but have no control over it. I've tried Legion, Tali & Mordin as the specialist before and Samara as the other team lead, but none of it works. I can keep everyone up except the one who goes in the tube.

what am i missing?

I have never once lost anyone, so long you use your brain

Fire Team Lead: Miranda & Garrus & Jacob only

Pipe Team: Tali or Legion Only

Shield Team: Samara(Morinth) or Jack only

Escort Crew Back: Doesnt matter, just send someone

When you start up the final fight, don't say: We have to Hurry(you raise the odds of someone dying)

Make sure everyone is loyal

Make sure your ship has major upgrades needed, Armor, Shields, Guns.

I now how 5 play throughs(playing every class in ME1, porting em over to 2 & playing), I have never once lost anyone yet, with this setup, ever.

1st playthrough, I sent Grunt Back, had Garrus lead both fire teams, Tali in the Pipe, Jack Shields. I took Miranda & Samara for final fight

2nd playthrough, I sent Grunt back, had Garrus lead both fire teams, Legion in the Pipe, Samara shields. I took Garrus & Zaeed for final fight

3rd Playthrough, I sent Jacob back, had Garrus lead both fire teams, Tali in the Pipe, Samara shields. I took Miranda & Legion for Final Fight.

4th Playthrough, I sent Jacob back, had Garrus lead team 1 & Miranda lead team 2, Legion in the Pipe, Jack on Shields. I took Mordin & Morinth for final fight

5th Playthrough, I sent Tali back, had Miranda lead team 1, Garrus lead team 2, Legion in the Pipe, Jack on Shields. I took Legion & Zaeed for final fight.

No one has died yet & crew has survived 100%

I'm sayin' i've done this before:

and Tali took a rocket to the face. Everyone was loyal and upgraded.

I'm just saying there's something else. Maybe it's random. Maybe it's based on something else you say. Maybe... dunno.

Still, as i had finished off the Human-reaper i saw Mordin and Thane lying on the ground and i was like "no... aw come on... wtf?!!.." They hadn't been assigned to any tasks whatsoever.

Please, a little help? Where did i go wrong?

I made sure i had the armour, shield and thanix canon upgrades. All of my crew were loyal, except Thane, cause i was forced into that "crew-being-abducted" situation before i was able to do his loyalty-mission.
That's another thing that bothers me. What determines when the reaper IFF-installation is complete? is it possible to delay it on purpose, or is it purely randomized to blow up in your face at anytime after you get it from the derelict reaper? Cause when you try to use the galactic map, as you all know, at some point you are forced into the crew being abducted and you absolutely HAVE to do the suicide mission, or else your crew will perish.

Distraction: Loyal Garrus or Loyal Mirranda.  Garrus is the optimal choice, though. Zaeed is horrible.
The last bit means your "defense score" for the distraction team wasn't high enough. You're better off sending a "weaker" character to escort your crew back to the ship: as long as they're loyal, they'll live.

What I did:
First Fireteam: Loyal Garrus
Tech: Loyal Legion
Second Fireteam: Loyal Garrus
Biotic: Loyal Samarra
Escort: Loyal Jacob.

Final boss party: Mordin and Miranda. 

Everybody made it out on my first try.

I had everyone but Jack and Zaeed loyal on my first play through, Paragon who choose to rush off right after the crew was abducted. Legion went into the vents, Garrus led the fire team, Thane escorted the crew don't recall who led the final fire team, almost positive it was Miranda though. Took Tali and Miranda into the final confrontation and had everyone survive.

Edit: only additional info I can think of to add, is Samara maintained the barrier and Tali was LI.

on my second playthrough used infiltrator renagade shep. only did loyalty for Mordin, Morinth, Tali, Garrus, and Miranda. also did not recruit Legion. I stuck with what worked for me first time, Miranda as leader both times. Morinth as shield, Tali as techie, and i wanted Mordin to live so he was escort. my final squad was garrus and miranda. only Jack died, i was hoping for maybe Thane or Zaeed to perish but just Jack. I guess she was the only squishy left...oh well

 Yes, ive done it!!! This is what i worked with:

Thane wasn't loyal, i failed his mission: dont ask :L

Taking that into consideration i didnt allow Thane to participate in any way;
Tali-Pipe
Miranda-1st Squad Leader
Mordin- Take back crew
Samara- Biotic Shield
Garrus- 2nd Squad Leader                Goodluck! :D

Actually if you check the character pages on the Mass Effect 2 website it states that Mordin is 50.

He's essentially the equivalent of a 80-90 year old man!  I guess that might explain why he "randomly" dies if you keep him in the squad.  I think the only surefire way to save him is to send him back with the Normandy crew.

That's relative age.  As in  Mordin being 30 is equivalent to a 50 year old human

Indeed. He is no soldier, he is former SpecOps and scientist. Escort jobs and such are exactly his strong side, not desperate firefights against hordes of collectors

GUTTED!! I just completed my third or fourth game having previously survived with ALL team members alive, and was REALLY trying for a 'perfect' run through. Went everywhere, completed every mission i know of, had 100% loyalty from EVERY team mate including Kasumi... and watched in horror as Legion, Zaeed AND Mordin ALL DIED!! Worryingly it seems as though a certain 'random element' means that you CAN lose team members no matter HOW well you prepare.
