Yes, it is possible, and kzelsama over at DA Nexus has done a wonderful job at it!
AFAIK, you need NewByPower's Blender Import/Export script (at least if you use Blender for the modelling), and need to tweak the hairs to sit on the scalp of the race. It's not something a newbie at modding could do easily, though if you're experienced at Blender/3DS Max/Milkshape/whatever, I don't see why you couldn't.

DarthParametric's guide is about as easy as anyone could explain it.
The UV map and face textures are just a guide. I.E., if you wanted to paint a flame around the left eye, you would use the face texture as a guide, and then paint the tattoo around the eye. Then follow the rest of the guide.
Eh... That was pretty much what the guide said anyways, and I really can't simplify it any more. He did a good job with the guide. You'd need a decent background with your image editing program, and know how to use the channel menu and stuff, as well as some knowledge as far as how textures in DA work.
I learned by trial and error... So all I can really say is read his guide thoroughly, don't skim... And
trial and error. When I made my first custom armor skin, I was bashing my head on my desk (literally) for days, but I finally got it working. It's not something you can just half-read a guide and instantly be a master. I've been doing textures since Oblivion, and I still suck at it.
If you really just can't get it... Ask for a step-by-step guide, or paint your tattoo design in some image program, and ask someone nicely to make it into a tattoo for you.
Tl;dr: Read DarthParametric's guide. Make your tattoo. If it isn't right, make it again until it is. If the whole thing gives you a headache... Well, you probably need to practice on more basic stuff before you tackle something more complicated.
