I'm really annoyed and disturbed when Shephard does that weird grimace expression upon handing Dr. Chakwas her Serrice Ice brandy.  Really weirded me out.

Moridin's Gilbert and SUlivan Song scene. (was laughing so hard i fell out of my chair)

Moridin acting a my Gynocologist! (playing as femshep)

random Salarian: Tell your assassin to aim for her head, she doesnt have a heart!

the most annoying lines are when your squad yells at you for shooting at them. almost invariably it is because "they" do something stupid and get in the way of your bullets, then they have the audacity to mock you. very irritating, devs should remove this immediatly.

The cigar smoking elcor on Omega has this one gem,
*not quite right I know*
Shepard "Let the quarian go or I'll break your legs." 
Elcor "With barely restrained terror: You are a tough negotiator Shepard."
The monotone in which the elcor says the line makes me snicker everytime.

I know this thread's old, but the game's expanded and I've got a couple annoying lines to share.



Donovan Hock: I'll ask your friend if she can wait outside. You understand, I hope.

Shepard: Care to explain why my friend has to stay outside?

Hock: I don't like the look of your friend, so she stays outside. Simple as that.

Shepard: If it'll make you happy, she can wait outside.



God... like two halves of a broken record...



And this is one that makes me cringe just because of the rhyming. I don't know why, but it REALLY bugs me.
