Think of the worst possible ending for Mass Effect 3.

=============SPOILERS!!!===============
Seriously though, worst scenario for ME3? Everyone your paragon has ever helped finally back stabs you, your romance option sells the Normandy out to the Reapers, Shepard dies a horrible unavoidable death at the hand of Protheans that aren't Collectors, ruining the continuity, Joker becomes the new human spectre wearing a combo tech and biotic power armor to counter his weak bones condition, and the Reapers are defeated by a simple computer virus EDI came up with, despite their being millions of years more advanced than humanity or anything its ever made. Then, as the credits role, somewhere far away, Joker's next threat opens its eyes...Zombie Commander Shepard, Eater of Worlds!!! Thus begins the next saga of Mass Effect, Commander Joker's Adventures!
Sadly, having said all this, I just realized that the part about the computer virus beating the Reapers is probably true. EDI is Reaper based....crap.
O, and @Mendelevosa: LOLs

The worst possible outcome are the news about Obsidian Entertainment taking over the Mass Effect franchise. I really hate them.
AAAAAAAAAAAHHHHHH!!
...
I'm sorry, I've just remembered how Obsidiian ruined Neverwinter Nights and Knights of the Old Republic, two Bioware games I absolutely loved.
OK, I'm done panicking, carry on...

I dread that this may actually come to pass.

Worst possible ending? Shep finds a magical deus ex machina machine, that was built in ancient times to defeat the Reapers with the push of a button. The universe is conveniently saved and every player feels sorely, sorely cheated.

Hah, that sounds like the end of every season of both Stargate SG-1 and Atlantis. Especially the sorely cheated part. :\\

The clues from ME 2, I think say as much in my personal opinion and either of those endings would suck big time but are probably likely unfortunately

To be fair KoToR II would have been an awesome game if it weren't for Obsidian being horribly rushed and having to push out the game as an unfinished mess.
Modders have finally finished the massive project to rebuild and finish KoToR II. The complete, finished game is amazing and much better then KoToR I in my opinion. If you still have the game you should google the KoToR II Restoration Project and play the game as it was intended.

Worst possible ending: Shepard gets knocked out after defeating the reapers, and he/she wakes up in Ferelden, as a Grey Warden in Ostagar, right as the battle begins.

Blasto the Hanar spectre DOESN'T appear to aid Shepard in the final battle.
Dont even joke about that, man. The universe is screwed without Blasto.

The Mass Effect Relays are destroyed and galactic civilizations comes to a standstill. Humanity is reduced to a few puny colonies and Earth itself, a cesspool of environmental degradation. Shepard wanders sets out on a one way trip at FTL speeds, never to return.

Obsidian ALMOST had a masterpiece in KOTOR 2. Man it was so very very close but they dropped the ball and haven't really done much up to the level of even a unfinished KOTOR 2 since.
It was sort of funny I was playing along in KOTOR 2 just in awe of how awesome it was and then about 3/4 of the way in you start going 'wait what just happened this makes no sense..." and it never really comes together again. Then you go read about what Obsidian had planned and it sounds amazing. I know people tend to blame the publisher but really...there is no excuse for totally butchering a game like that. Especially one that had the potential to be so amazing.
Anyway the worst possible ending is that the Ewoks defeat the Reapers.

Shepard must resort to picking up a bow and sword, learn magic moves taught by a rogue elf and reassemble a 'clan' of warriors taken from such races as dwarves, hobbits and orcs. You spend the last moments of the game fighting an evil wizard.
THE END
