On the topic of who might die; I think BioWare is going to put full detail and work into all the party members who survive, except say.. ones who planned to leave anyway. Samara has her Justicar oath to uphold, although if you were loyal with her, maybe she'll decide to stay?
Thane might die.. Depending on the time-frame, but if the game is set with close proximity to the first, he could still be alive, and then have a big role again.
I don't think BioWare is going to cut every single party member they've spent years developing down to cameos. People forget that the cameos in ME2 (Liara, Ashley, Kaiden) are
only cameos because they need to survive for ME3.
When they are working on ME3, there is no ME4 to plan for after the finale, so they don't need to worry about that. Hudson has specifically said they are going all out, to make sure it's a story unique for everyone, and drastically so. Maybe I'm just being delusional, but I can't seem them throwing away all that work. I think they'll fully detail all the characters (at least the ones staying with you, as I said).
They don't have to worry about a new graphics engine, or a combat system this time, except for minor tweaks. It's all gonna be about wrapping up the story. Cutting out all your major characters seems like a terrible way to do that, and I can't see it happening.
I don't think they'll have to artificially bring anyone back to life. If they died in your game, you just don't see them.. But that major content is still there for those who did keep the character alive. They are trying to do what no other game has done, and truly make it a unique experience.
