The replacement doesn't have to be a squad mate. It's not worth creating new squad mates JUST if a previous one died. If they do that, people are missing out and have to have a playthrough in which x character died. The character replacing can be a squad mate, like the Krogan leading the Urdnot clan instead of Wrex. to even import Shepard to ME3, you need to have at least 1-2 squad mates survive. Add Liara and Ashley/Kaidan to that list. That's at least 3, which is enough for a squad. Bioware could of course add a couple new squad mates independent of who lived or died in ME2. Then there's at least 5, which is about the number ME1 had. If Tali died in your playthrough, another Quarian NPC can take the reigns.KainrycKarr wrote...

IF X character died, they are replaced by Y character as your squadmate for ME3.
That's pretty much what I'm saying.
