Different chiralities does indeed mean completely opposite. Mirror images in fact.
Human (and all other earth life) amino acids are generally portrayed as a carbon atom bonded to four other groups: an amine (NH2) group on the left, a carboxyl (COOH) group on the right, a hydrogen atom above, and a carbon chain below. This would be described as a laevo-amino acid (or S chirality).
Since the canon seems to state that quarians and turians are based on dextro-amino acids, that would mean that, in their proteins, the carboxyl and amine groups in their amino acids are on opposite sides, making them a mirror image to human amino acids (R chirality).
Essentially, it's like comparing a left hand to a right hand.
On an unrelated note, this isn't how I imagined my first post on the ME2 boards would turn out.
I conceed to the actual Chemist, but still, you'd have to agree with me that the genetic difference only prevents them from having children, and they have to use protection to have sex, correct?
I don't think anyone actually knows...Except maybe for the actual creators of the Quarians and that's if they bothered to show enough forethought to think that far ahead.
