I just finished my second playthrough, it was on insanity, and I was a renegade. My first was on normal and I was a paragon. Both were so far apart from each other its crazy. Let me recap:
Paragon/Renegade
Playing through as a paragon made Shepard feel like a very influential and caring character. You could solve basically any argument or situation with your charisma, and you felt good about it. Also, no scarring. Play as a renegade however, and you get a completely different experience. Playing as Shepard makes you feel like a total badass; pushing people out of windows, punching reporters, stabbing people in the back with welding tools. In addition you get pretty bad scarring on your face and red eyes (if you go far enough).
Difficulty
On normal I felt in control of any battle or situation, and wasn't really scared (playing as Soldier). I could rush into a battle if I wanted, being careful, and still come out on top. Play through on insanity on you aren't as cocky. You have to actually utilize cover, there's no running in and pumping bullets into anything that moves. Your powers become much more of a weapon than before and you're constantly watching your health. Also, ammo must be conserved. Every shot's gotta count or else you might end up losing. Every fight could easily be your last if you aren't careful.
Exploration
If you explore everywhere (planets, missions) you'll almost always get some kind of reward, whether its uprades, credits, or minerals. There a lot of side missions hidden on the many plantes as well, and some of them are different from the usual 'shoot everybody' mindset. Some of them have you following a YMIR Mech and powering it up so it can mine minerals, while others have you restoring shields to a remote research base. They aren't anything special, but they're definitely a welcome change to the usual missions. Now if you do no exploration at all, in planets and missions, you won't be a happy camper. Many weapon, armor, tech, and biotic upgrades will be missed, therefore making your character a bit weaker. Same if you don't probe for minerals, you won't get many upgrades at the research station on the Normandy. This will tie into the next section.
Endgame outcome
Upgrades and loyalty play a very large role in the outcome of the game. If all upgrades are researched the Normandy won't suffer any damage in the last mission, and it will obliterate the Collector ship that gets sent at you. The more loyalty missions you complete, the lower the chances of your team mates dying are. When all loyalty missions are completed, all Normandy upgrades are obtained, and the correct decisions are made, none of your crew or Shepard will die. When upgrades aren't obtained the Collector ship will blow straight through your shields, killing multiple team mates if their loyalty missions aren't completed. When you go at the Collector ship in an attempt to kill it with unupgraded guns, nothing happens and another team mate dies. Go on the Collector base with no loyalty missions completed and your whole crew will get decimated. With everyone dead, only Joker will be at the Normandy to pull you up. Being as frail as he is, that obviously won't work.....so Shepard plummets to his death aboard the Collector base. And the already slim chances of him surviving the fall are wiped out when the Collector ship explodes/radiation purges (depending on what you choose to do). Very sad ending!
I just wanted to give kudos to BioWare for being able to make 2 playthroughs on the same game so very diferrent. Not many games can do that, but Mass Effect 2 obviously can. The game is amazing and I'm looking forward to any future DLC/Expansions that are to come.
