When building your squadmates, is it benefitial to fully upgrade their passive class ability (Salarian Scientist, Cerberus Operative etc.)? and after that  when picking their final upgrades for powers, most have the option to increase area of affect, and the other one increases potency/power, which do you typically choose?

Depending on your class or needs...
1) Overload and Warp are extremely useful and helpful. Reave as well. I'd say essential as you go higher in difficulty. Miranda and Garrus are a good combo when taking care of Shielded enemies, while Miranda pairs well with Armored ones.
2) If you're looking for Crowd Control, the biotics are good, (Pull/Push/Singularity). I would have said Shockwave but you'd generally need it if there's way too many enemies crowding you (Hi Husks), but it has varied effects from long distance. Jacob (Pull), Samara (Pull/Push) and Jack/SZ are good in this department.
3) Warp Ammo and Disruptor Ammo are good. I generally don't use Inferno Ammo(sp?) or Cryo Ammo, since Warp and Disruptor pretty much covers my needs (Disruptor for Geth/Mech. Warp for anything else).
4) General consensus says that AI Hacking isn't very good (Sorry Tali/Engineers), because it only works with enemies with no shields/armor, by then, you'll have an easier time just putting more bullets into the thing and get it over with.
5) No clue on Inferno Grenades, never used 'em.

Anything else I recall when I play ME2 again, i'll post it up.

I usually choose a pair that I will use for pretty much the whole game and choose their focus based on that.

For example I like the Thane/Garrus combo, in that I boost Thane's warp and Garrus' overload asap along with their passive upgrades. Then I branch out and put some in their ammo types/other abilities.

Really it all depends on what class I am, that combo is as an Infiltrator. As a Vanguard I have Legion/Miranda. Not that I am a master at this game, but it's worked well on hardcore for me. I just try not to get overlapping abilities unless they're really useful like warp.

I like Thane and Legion/Tali if I'm a soldier, They handel long range and tech/biotics and I cover them. I noticed whenever I play with Garrus, he dies a helluvalot, is it that his AI isn't good or just he doesn't have enough health/shield.

I always have each teammate max their class talent.

The only squadmate for whom I max the passive talent ASAP is Miranda, as hers boosts the entire squad, not just herself. I don't rely on them for damage dealing as much as for complementary skills (and to draw fire away from me as needed).

When playing Engineer, for instance, I have great powers against shields and armor, but I bring along Miranda or Thane to Warp enemy barriers, and maybe someone like Jack, Samara, or Jacob for Pull Field to help control hordes that might try to rush me.
