That sounds very similar to the glitch encountered during Grunt's mission where all enemies are killed, yet the keysone does not get activated for the next wave. Maybe it could be a clipping issue(I know this game has enough of those) where an enemy is somehow thrown out of the playble area(possibly from a biotic power?).

Not sure if the OP is still updating the post, but there has been no mention of this bug.

Abstract:
Mission: Dossier: The Warlord
Location: Finale Battle with Jedore
Cause: If you lure an enemy behind the door leading into the battlefield with Jedore, and lock him inside without killing it, then there will be no way of finishing the battle.
Occurance: Any time you lure an enemy back and lock them at the stairway.

Full details:
First you need to understand some of the layout of finale battles. Usually theres either a door, cutscene, or some one way path into the boss battlefield.

For Dossier: The Warlord the choice was a one way door lock. The door would lock behind you after you and your team has successfully crossed it. This door would then unlock after all enemies including minions have been killed.

Now the problem is you are able to start the battle while behind this door. To do this you simply by stand on the doorway so the door doesn't close and shoot at Jedore to start the battle.

If you stay behind the door while the battle has started, your allies would move ahead attacking the enemy krogan(s) and YMIR mech. Eventually one of the krogans would be attempting to move up towards your position behind the door to the battlefield. If you let them successfully move pass the door to the stairs, and run out of the stairway, the door behind you would lock, thus trapping the enemy on the stairs. And thus trapping you in a battle that can't end.

(PC) Bug during Tali's loyalty mission: Treason.

After first arriving on the Flotilla and getting the update to proceed to the hearing, if you hit space to talk to Admiral Shala'Raan before getting close enough to activate the cutscene, you will skip the entire preliminary hearing.

Shala'Raan will converse with you as if the preliminary hearing already happened (starting with Tali's "You set me up!" ... which was just the beginning of my confusion), and then your journal will update you to head to the Alarei. None of the other admirals will accept conversations, and Veetor and Kal'Reegar's names will appear, but they will be invisible and non-interactive.

Everything after that works fine, but you miss out on Paragon/Renegade points, and certain changes in the later final hearing due to not being able to talk to some people after the first hearing.

Saddest part was that I wasn't even sure I missed anything until my second playthrough, and triggered the hearing properly, much to my surprise.  Tested and reproduced.

Obvious solution is to not attempt to talk to Shala'Raan until after the first hearing triggers.

I have had this glitch and it seems to not effect everybody but. When I did my first play through, which I did on insanity with a female infiltrator shep I did not get my achievement at the end of the game. I did cancel after beating it the first time once I realized what happened to my crew due to my delayed action and so I loaded back. Now it seems I am locked out of that achievement as each time I beat it with that shep it doesn't show up. I have yet to try to beat insanity with another shep but insanity is really hard I would prefer to get it without having to replay the entire thing on it.

Not sure if the OP still updates, but here are the bugs I noticed while playing ME2 for the first time. Already mentioned ones appear in Italics.

* All Iridium crates in the final battle during the Tali recruit mission only give 1 Irridium.
* Flying Platforms on Collector ship (and possibly base?) can get stuck with combat drone if you fire it at the base of the platform. Forces the player to reload a save.
* Combat drone triggers Talia father death scene if cast at the door where the Geth spawn during that mission, even if all Geth are still alive in the previous room. Very sudden.
* Rich-depleted scanner.
* Minos Wastelands chart- Charts is capitalised (not with the others).
* "Enough resources for project" message does not take into account reduction by skill.
* Human colony; First encounter with Collectors- You can walk on the wall by walking to the crate on the right.
* Couldn't depart from Mordin's mission after saying we should stay (game appeared to think I was in combat). Could after loading the quicksave point though.
* Urz (varren) returned back to life after Mordin's loyalty mission?
* At Kenn's Salvage (Omega), Zakera Cafe (Citadel), Sirta Foundation (Citadel), Saronis Applications (Citadel), Rodam Expeditions (Citadel), Serrice Technology (Illium) and Baria Frontiers (Illium) you can purchase "nothing"
* N7: Javelin Missiles. Completed questlog mentions I saved the colony, while I chose to save the spaceport instead (post-mission report said correctly though)
* Level 24 is reported as gaining level 0? (Firewalker: Vulcano Station was were I gained it incase that has to do with it).
* Hammerhead makes damaged sound throughout all other levels because I exited first planet damaged. That was quite annoying.
* Firewalker: Geth Activity. Final node got ignored. Unsure wheter that was because of "Frozen" status or because the computer was talking, overwriting teammate end convo. Did work as it should second time (Cold, no comp convo activate at the time).
* "Packages for Ish" sidequest. On Citadel Station the log's we?re should be we're.
* Leave Illium movie not skippable like the other commercial planets
* Leave with the IFF: Incorrectly mentions "Reaper IFF core" instead of just "Reaper core"
* Heretic Station 'activating defenses' topic has VO/ST mismatch, and Legion repeats himself too once.
* Heretic Station; loading the quick-save of the defense, use the console and say 'Hold on' still triggers the assault.
* Shepard (male) to EDI: "Where do they get that kind of inc-" (line is cut short). Only line in the entire game for me, and repeatable so pretty sure it's not the same issue some others have repeatingly with random lines.

Told her "not right now" on mission.



Walked right through the scanner without the "You're dead" dialog.



Stopped by Baily's desk. He "fixed" my problem anyway.



Couldn't get to the Presidium. Not an option at the transit station.



Walked back out through C-Sec. Got the "You're dead" dialog.



Walked back out to talk to Baily. My "problem" was solved and didn't come up.



Went back to the transit station. Went to the presidium as normal.



I was playing soldier class.

I am having a issue sometimes when I take cover behind a like a stack of crates and boxes even if theyare 2 high that for no apperent reason my charater just goes to the top of them and cant move. leaving me wide open to being shot lol!!!! Is there anyway to fix that it gets annoying after awhile Esspeaiclly if youu had not saved and it just randomly happens

If you choose the 'no time' dialogue this is normal.

I did get something similar though.

If you say 'take your time', then try the door, then an object that triggers Jack to speak, it instead cuts to the shuttle scene when jack tries to talk.

Example: Touch the window and jack speaks, then touch the door, now touch the wall with the blood stain and instead of speaking about the stain, you will wind up in the shuttle.

Not a bug as such, or perhaps it is, but in some cutscenes Shepard will be holding an Assault Rifle even if you cannot equip them. I think it happens, as an example, in at least one cut scene on the Dossier: Archangel "chain".

Will try get more information on it.

Glad to see I'm not the only one with the Ish quest issue. Another one that came up for me my first playthrough, while helping Archangel/Garrus defend his position on Omega, I killed everyone but nothing happened. I had to reload a previous auto-save and kill everyone again before it worked.

p.s. this thread really should be stickied.

On the mission Thane: Sins of the Father, immediately after talking to the dock boy in one of the rooms the game locks up; not able to move shephard and Thane keeps saying "I'm loosing him". Needed to dashboard.

I was on the mission: Recruit the Krogan. After he opened the door I killed more Krogans then all of a sudden my character floated up in the air and I was stuck there!

With regard to imported ME1 saves: Is it normal for your imported character to not carry over his rank? In my final save game from ME1, I was a Level 56 soldier. Yet, when I import this character into ME2, he starts out as Level 1 or Level 3. Do you not retain your rank from ME1? Or is this a bug?

Destroy batarian terrorists:
After redirect missiles does not complete the mission. Only if you replay it again.
This has happened every time I redirected a missiles in the first time.
When I deciphered codes missiles movie "frozen" and not was continue.
Or, the movie did not start, I could walk on location, but get out of it was not possible.

- Task: False documents. Citadel.
Two Azari appear only after the passage of mission - Reapers system: "Native or Strange".
This is by design? Prior to that, they never appear.

- Task: Illium.
The officer Parazini requested assistance exposing the Smuggler.
If you notify Smuggler, experience points for performing tasks are not issued.

In the first time will fly by Illium showing the movie, but if you depart and return movie is not shown.
The result: Displays the movie is only in the first Illium visit.

- Task: Mining the Canyon.
Activate YMIR Mech to search for minerals.
When recharging the Mech he not opening, but moves in the folded position. Or will open and close the back, and so continue to move.
Panel activation of the robot is not on it, but somewhere near "in space".
It disappears focus, and you can not activate it.

To return the focus should go to the menu (press Esc) and return to the game(again press Esc).

Loss of focus (no possibility of activation) on items, doors, characters is present throughout the game.
If not immediately activate the item while it is lit in blue
possibility of activation disappears and you have to perform the above described actions.

No one experienced this?

- Mission: MSV Strontium Mule.
The door to the bridge must breaking.
But if you click Cancel during a breaking, the door opens itself.

- The mission of The Engineer.
Recruit Tali'Zora.

You need to bring explosives.
When the first take explosives, appear Troopers Geths headed by Geth Prime.
They are difficult to win, but if you run to the second explosive, without getting involved in fight, then Geths Troopers "incredible" way will be lost.

This is not a bug, but should also be noted it.

I've already completed Grunt's loyalty mission.  I then killed the pyjacks.  When I went to Ratch to tell him and get his store discount, the only new thing was that I was able to buy the pyjack meat from his kiosk and give it to his pet.  Now he won't talk to me and give me the option to get the discount.  It still says in my journal to talk to him in order to get it.

Is there something else I can do?

I'm playing on the xbox 360.

UPDATE: After completing Mordin's loyalty mission, I was able to talk to Ratch about the pyjacks and he gave me the discount.
