I mean to one of those non standard ones, I see a few but I dont know hows!

I believe it has something to do with changing the portrait file in your Games>Bioware>Dragon Age>Characters folder. If you look at the profile of those people with custom avatars, you'll see that they've "recently uploaded a new character from Dragon Age" and the character is their custom avatar.
