In single player games I always name myself Josef (my real name) unless I plan on roleplaying in a way contrary to my character or because my/the PC is female in which I name her Avalessa (an name I use in MMOs).

In MMORPGs I use Saija/Schurge... I didn't find out that Saija was actually a girl's name until after I had been using it for awhile so now I still use it. Schurge is a mispelled version of 'scherge' which roughly means 'myrmidon' in German. I know thats unrelated... but it seems made-up names are all the rage.

So I have three Josef's... But two of them arn't making it beyond Ostagar just made them to experiment with other openings, along with a generic female dwarf who isnt making it past Ostagar either.

I think using your own name gets you into the story more... besides I like my name... I don't want to be Zanzibar von Vanvan.

@ Schurge - I can never use my own name in an RPG. That feels too much like a self-insert, and I really try to craft different characters for each play. It makes it more interesting to me. Plus, I'm a woman in RL, so it would be awkward playing with a male avatar with a blatantly female name.

Quan'Chi - Mage

This was my Dark Jedi Knights name in SWG, I spent 6 months leveling him to Jedi Knight. The day after i finished leveling him and reached Knight status Sony released the NGE (patch that killed the game). All that work thrown in the trash . So anytime i create a magic oriented character in a game i use this name and try to create a player similar to him (i cant let go lol).

Just saw this old discussion and even though the last response was about a year ago I still wanted to give my names as well:

1) Elven mage female: Nienna
2) Human mage male: Declan
3) Human noble female: Bridget
4) Dalish elf male: Vartan

For the dog I choose whatever comes to mind at the time (e.g. Gladiator, Hercules, etc.) but I can't remember which character has which dog, except for Nienna, for the storyline (and a bit sentimental reason) I called the dog 'Neralos', after her dead fiancee.
