Well, I saw an interview where they told you that not upgrading your ship would have consequences. Thats an official pregame source though, not a forum guide. I would have done it anyway. Vague enough to count!
Upgrading the ship seemed like a no-brainer for me. Especially after what happened to the first Normandy. I knew that would be important in the endgame.
Yeah, it's pretty obvious upgrading your ship would do something pretty damn important. You look at those and say "huh, I know I'm not going to be controlling any ship combat, so obviously, this determines the outcome of a scripted cutscene. "
It's a no brainer, really.

on my second playthrough.. I decided my renegade shepard would be too impatient to try to probe the planets so I didn't do any scanning.. I saved resources for the vitals: shields, armor, and weapons. and spent the rest on prototypes and any weapons I could still afford from what I found during missions. I know I had less than half of the research items done. I recruited everyone and did the loyalty missions and went right away for the suicide mission after I did the IFF mission. Didn't lose anyone... My 3rd playthrough... can shepard survive if I only do a single tech, biotic, leader, and a soldier loyalty missions..

It's really just common sense. The only permissible mistake I see people making is putting Zaeed in charge of a fire team. He *was* the leader of the Blue Suns, after all.

First try. No casualties.
It's not like I'm sad that everyone survived, but I think it would of made the story better if I had at least lost someone. It wasn't much of a suicide mission.

If i got it, i would've had every achievement (all 1015 g) in 1 playthrough. But then I took too long ,didnt know it depended on that too

It really isn't that hard. For completionists, the only real hurdle is choosing the leader but even then common sense sort of dictates Miranda or Garrus as the most experienced/logical. It probably would have been better if I had lost someone though on the first run... it's too hard to knowingly screw up the mission just to get someone killed.

I did. Just paid attention to the character descriptions, as people have said, and sent Mordin back to the ship hoping he'd be able to come up with a better seeker countermeasure which saved him from randomly buying it the way he did for a lot of people. Currently stuck with random people buying it on my second playthrough with no idea why.

I almost got No One Left Behind on my first playthrough unspoiled, however a trick of fate and time stopped me. Essentially, I did Zaeed's personal quest too early and didnt have enough paragon to charm him, thus I could not gain his loyalty and thus he died. He was the only one who died.

I did. I did all the loyalty missions because I knew that was a big deal, and then listened to the descriptions (tech expert, leader, super biotic). By luck/whatever I did Tali, Garrus, and Jack. I was glad to have Jack as my forcefield, because Samara was better as a teammate.
I agree with prior posters who have said it's hard to go back now and purposely mess it up. I dislike Jack, and so I'm annoyed I have to pick her up at all. She won't get the loyalty mission and I'll probably send her through the tunnels. But other than my interest in knocking off Jack, there's not a whole lot of incentive to run through the suicide mission again.

I got the achievement on my first play through. I made sure I got every ship upgrade and gained everyones loyality. And I made stopped and thinked for a bit when I had to choose which squadmate did what. I had Legion in the vents, Garrus lead the second team each time, and I had Samara create the biotic barrier sphere to protect from the swarms.

Also managed on first try without getting anyone killed. Had all ship upgrades (including medbay) and most other upgrades (some prototypes were missing). Legion went through the tunnel, Miranda led the first team, Garrus the second, Samara did biotic, Jacob escorted the crew out.

I'd been spoiled from the tech-forums and from a friend only to the extent that it's possible for people to die no matter their loyalty. I was sweating bullets during every cut scene with my squads. In the end, everyone lived and I got the ending I wanted for my carry-over save for ME3 on my first try.
Now it's time to wait a couple years till the end of the Trilogy, heh. And now I can browse and read these forums without ruining something for me.
Here's what I did late Sat night. I waited to do the Derelict Reaper mission for the end after ALL the upgrades and sidequests were done. By that point it was easy to see that that Reaper mission was ushering in the end part of the game after you get the IFF.
My Shep was a Soldier with the Revenant LMG. All my companions were loyal and I went to save the crew ASAP.
Here's my team set-ups and the reasons behind them.
Vents: Legion
--Geth super-hacker who doesn't need atmosphere or gravity... and who knows what could happen in the vents
2nd Fire Team Leader: Miranda
--Her overload and warp would be good to keep others alive as when I had her in my team
My Team: Garrus & Tali
--Facing down a threat with my old friends like old times. Garrus' Concussion shot and Tali's Combat Drone... very helpful.
Escort for Crew: Samara
--A Justicar would help those in need
Biotic Barrier: Jack
--The "most powerful human biotic in the galaxy" seemed qualified for the task
My team: Garrus & Tali
--Again with my old friends. To Hell and back.

--Again with her skills and powers
Finale squad: Garrus & Miranda

I was able to get all but 4 achievements on my 1st play.

Yep. First play-through, perfect.
I wasn't surprised to be honest. I did all the loyalty missions, purchased all the upgrades, and used common sense in choosing specialists. Easy. Second play-through I chose Thane to lead the distraction team and he died, which made sense to me.

I got it done. I sort of took a hint that the reaper Ident was a set up for the final mission before I went on it, and pretty much scanned out the rest of the galaxy in advance. It was kinda easy to figure that Garrus and Jacob were the two best leaders and Legion was the best to solo the conduit.

Miranda couldn't make a good biotic field to save her life. Or Jack's life, anyway. She was eaten by the collector swarms. Reloaded with Samara, and it was all good.
