Kelly, and some extra crews from the normandy (like those useless sacks that did nothing but talked/complained about the food)
in my defense... i didn't know there was time pressure... And i chose Jack over Miranda for the make a choice part, and never used either and it all worked out fine.
tali = tech expert
garrus led team with jacob
garrus escorted back chakwas
samara was shield person

Who to crawl through the vents and open the door? Legion was synthetic... he had the best chance of sneaking through undetected. AND he could hack like a chainsaw on steroids.
Who to lead the fireteams? Garrus showed he had capacity for leadership by leading his own team once, and Miranda has leadership qualities, even if she IS a b*tch. I used them both, first Garrus, then Miranda.
Who to create the biotic shield? Well Jack had raw power... but I chose Samara instead. She has had hundreds of years of Asari biotic training, more likely to be able to keep up concentrated focus.
And finally, I sent Zaeed back to guard the rescued crewmen. He was a grizzled veteran, used to working on his own, and I trusted him to get it done.
I chose all of these through logic and a bit of instinct, and before even beginning the mission I had full ship and squad upgrades and loyalty. I took no chances, and ME2's final mission got itself OWNED!
Though near the end when Tali started sliding off the edge of the exploding platform I had my heart in my throat... but her hips prevailed!

I also failed to get Zaeed's loyalty, and lost Jack's too. However everyone still survived the mission for me - Jack and Zaeed actually turned out to be a surprisingly fun squad combo (but still took Shepard's most trusted comrades, Tali and Garrus, on the final run, for old times sake).
(Btw made my own choices re squad composition and switched around a bit just for variety, but couldn't say was totally 'unspoiled' - knew that the IFF mission would trigger the end game).

Some upgrades require prior upgrades to be done first, but iirc this doesn't apply to ship upgrades.

1st leader: zaeed(yeah i kno)
these 2 resulted in garrus' death
biotics:jack(her her story arc is how shes stoopid powerful biotic)
2nd leader: zaeed, again(yeah i kno!)
this got zaeed killed
escort:no1, was doing renegade char
door:i think i left grunt to hold it, and that he died
squad:miranda and thane
mordin and tali were dead
i had everyone loyal
so yeah, fail lol

Garus led the 1st team, and Legion did the hacking. Garus has combat leadership experience, and Legion is a geth so even if I lose it...well it is expendable being synthetic and haveing a back up someplace likely anyway. Shepard, Samara and Jack blew our way into the place...with me cursing that damned mechanic of opening valves.
Send Thane back with the crew since well he is overall leathal at all ranges, against all targets and this is right down his alley as he is a loner and he is the least useful in the team since he is a loner. Used Samara just cause actually she volunteered and I saw no reason to change that. Shepard, Jack and Tali were the one group while Miranda (keep her ego in check) lead the other.
Shifted back to the group of Shepard, Samara, and Jack for the last bit, I think I left Garus in charge again. Jack didn't go over the edge, the reaper discovered what happens "If Shepard has a rocket launcher" and the end cinematic was a total shock and surprise I don't even know what got the reaper since I think I was shooting a collector assasin with the sniper rifle when it triggered.

obviously the hacker you send through the vents is legion, a specialized super-geth who hacks through firewalls in your ship while standing there effing talking to you...
if you love your squad you put garrus in charge.. he led his own squad of 12 guys very well and felt so awful when he couldnt protect them from that traitor so obviously he's not only a beast in combat but you know he would take care of everyone.
samara ranks near-asari matriach in terms of biotic ability.. which is almost as good as it gets so she takes you through the bugs. i guess jack works too but i didn't pick her because she's supposed to be one of the most powerful HUMAN biotics.. and humans arent the level of asari as far as biotic ability goes.
the only questionable decision i made was letting zaeed lead my grew back to the normandy.. but i figured even though he's kind of ruthless, he's still a good leader and just a beast. it worked out, no casualties
so yeah everyone lived my first try... the only problem was after i got the IFF and my crew was kidnapped i didnt go rescue them immediately.. i did a couple more missions so by the time i got there kelly was dead, etc. i corrected that on my second and third playthroughs though and everyone lived.
i had read posts on here with mordin dying randomly though... so there could still be a little luck involved, but idk because so far i'm 3/3 no squad members died. maybe those people didn't upgrade mordin's omnitool? i think for the people who get upgrades, you have to upgrade them. like jack's biotic ability and grunt's health, etc.
i'm going to experiment a little on future playthroughs with not getting certain people's loyalty, etc. i'm probly going to play through this game like 20 times lol. for now though i'm going to replay me1 a few times, experimenting with different decisions, backgrounds, etc.
still my hypothesis is if you actually think for a second and you have everyone's loyalty and upgraded properly, everything will run smoothly.

Miranda for 2nd team leader both times
Tali in the vents, since she is a Tech expert and thats what they were after,
Samara for the biotic bubble, they basically suggest her or Jack,
Jacob escorts crew back.
Rolled with Garrus everytime, Grunt, Legion and Thane as the 3rd member in each section.
So, yeah I did that first time also....nur nur!

Well I guess I sorta lucked out with Garrus I just presumed he would be a good leader after leading his team on Omega.
I picked Garrus for the same reasons. Unfortunately, I did lose Miranda and Jacob. I had thought Jacob would be good for the vent mission, namely because he volunteered. I also wasn't sure how well it would turn as a whole for any of my party, because Miranda, one of the more influential members of my crew, wasn't loyal. As such, I didn't want to risk losing Tali or Legion (characters I genuinely liked, and felt I might need later on) in the vents if Miranda did something stupid. Poor Jacob.
Miranda was just sort of dead at the end.

I sent Legion through the vent. Used Miranda as a leader, and Samara for the biotic. Grunt and Thane were with me the whole way dealing out the damage on the hardcore level.
