It wasn't too hard, though I came extremely close to sending Thane through the shaft because of his vent work on Ilium.
Who got "No One Left Behind" on there first run, COMPLETELY unspoiled.

I did with no guide or help. Had full loyalty, played as paragon and had all the important upgrades recommended by team.
Legion through vent Garrus as leader. Then Miranda as leader and Jack as biotic. Samara escorted out the survivors. Tali and Miranda for the final boss.
Easy tbh.

I did. Saved the crew, too. Completely luck on both counts.
Legion in the vent, Jacob as leader both times, Samara the biotic, Grunt the escort and Miranda and Tali with me.

I did, wasn't hard or anything. Every upgrade was had, every member loyal, every choice had a really obvious answer. The only time where is seemed like anyone was going to die was when we all fell at the end. Beyond that, Virmire still reins as the most suicidal mission in the Mass Effect story.

I saved everyone the first time through, the Normandy crew and the team, pegged lvl 30 as well during the finale. Upgraded everything, got all loyalties and did all the side missions before the crew got abducted. I was nervous and waiting the whole time for people to start dropping like flies but it never happened. Sent Legion in the ducts be cause he's like the Data of the Mass Effect universe, he can do anything. Garrus for the first fire team, Jacob for the second, Zaeed to escort the crew and, SuZe for the biotic field.
Picking the right people at the end wasn't hard, you just use the cues that the game gives you. If you listen to what people say and have reading comprehension then picking the right people for the right jobs isn't hard.

Sorry if someone brought this up, but I didn't see it skimming through (and there is no search...)
For the people experiencing random deaths, did you put Jack under Miranda's command? I don't know if this has an effect, but I made sure to avoid it and got everyone through first time.
Tech Tali, Leader Garrus, Escort Jacob
All but Zaeed loyal.

That's exactly how it was for me. I did everything to prepare for the suicide mission but was still on the edge of my seat the whole time, wondering what seemingly insignificant detail I'd missed that would get somebody dead.
The worst part of all for me was seeing Tali and Miranda under that rubble. I was sure they were dead and I was like "whyyyyyyy?!"
Even the jump to the ship at the end. I didn't see how I could possibly die, with everybody else living, but I had flashes of Shepard falling to his death right up until the point that Tali (who happened to be his LI) caught him.
